摘要
目的:探讨螺旋CT容积扫描结合多平面重建(MPR)在对腹膜后神经源性肿瘤诊断中的临床应用价值。方法:21例经手术和病理证实腹膜后神经源性肿瘤行螺旋CT容积扫描,结合MPR征象进行对比分析,观察肿瘤的位置、大小、密度、形态及与邻近结构的关系。结果:21例腹膜后肿瘤,良性17例,恶性4例。良性肿瘤大多圆形或类圆形软组织肿块,密度均匀,主要呈膨胀性生长,边缘境界清楚;而恶性肿瘤体积大且不规则,混杂密度,呈浸润性生长,边缘不清,多累及周围组织血管。腹膜后定位征象:肿瘤多沿椎旁交感神经链分布,于脊柱旁、肾周、结肠外后方及骶前区多见,推压或包绕邻近器官。结论:螺旋CT容积扫描结合MPR能克服常规扫描方式的限制,获得直观、清晰的图像,准确的显示肿瘤本身及侵犯周围组织、器官的情况,明确肿瘤累及的范围,对肿瘤的定位、定性诊断具有一定价值。
Objective：To investigate the clinical application of the spiral CT volume scanning combined with multiplanar reconstruction（MPR） in the diagnosis of retroperitoneal neurogenic tumors.Methods：21 cases with surgically and pathologically confirmed retroperitoneal neurogenic tumors undergone spiral CT volume scanning were studied MPR signs including tumor location,size,density,shape,and neighboring structures were analyzed.Results：Of 21 cases of retroperitoneal tumors,17 cases were benign,4 cases were malignant.Most benign tumors were round or oval soft tissue masses with homogeneous density,clear margin,and mainly expansive growth;most malignant tumors were large in size with irregular shape,mixed density,invasive growth,unclear margin,and mainly involving the surrounding tissues and blood vessels.Signs of retroperitoneal location：tumors distributed along the paravertebral sympathetic chain,at perirenal,paravetebral and presacral area,and at latero-posterior area of colon,with compressing or encasing of adjacent organs.Conclusion：Spiral CT volume scanning with MPR can obtain intuitive and clear images,of retroperitoneal neurogenic tumors including the invasion of surrounding tissues.It has certain value in localization and characterization of tumors.
