摘要
目的:探讨髂动脉(IA)闭塞及侧支循环在多层螺旋CT血管造影(MSCTA)中的表现。方法:11例经MSC-TA确诊的IA闭塞患者,采用容积再现(VR)血管添加技术(AV)对侧支血管进行选择性重建,分析侧支循环的表现。结果:11例IA闭塞患者发生在左侧9例、右侧2例,均合并同侧髂内动脉闭塞,6例合并同侧髂外动脉闭塞,1例合并对侧髂内动脉闭塞,1例合并对侧髂外动脉狭窄。参与侧支循环的动脉包括患侧腹壁动脉11例,第10~12肋间动脉11例,第4腰动脉10例,第2腰动脉1例,骶正中动脉及直肠上、下动脉1例,双侧股动脉间分支6例,股动脉与第4腰动脉之间1例。结论:MSCTA能清楚显示IA闭塞及主要侧支循环情况,并能对患肢侧支供血情况进行初步评价。
Objective:To investigate MSCT angiography(MSCTA) in diagnosing the iliac artery occlusion and its collateral arteries.Methods:In the 11 cases of iliac artery occlusion confirmed on MSCTA,the volume rendering(VR) with adding vessel(AV) were reconstructed to show the collateral circulation arteries.Results:Of the 11 patients,9 were left and 2 were right iliac occlusions.All cases were complicated with ipsilateral internal iliac occlusion and 6 had ipsilateral external iliac occlusions.One was complicated with contralateral internal iliac occlusion and one was complicated with contralateral external iliac stenosis.The collateral circulation arteries included:arteria epigastrica(11 cases),arteria intercostalis(11 cases),arteria lumbalis(10 cases),arteria lumbalis(1 case),arteria sacralis mediana(1 case),arteria rectalis superior(one case),arteria rectalis inferion(one case),bitateral arteria femorlis(6 cases),and communication between the arteria femoralis and fourth arteria lumbalis(one case).Conclusions:MSCTA can show the collateral circulation artery clearly after the iliac artery occlusion which is helpful for evaluating the blood supply of the affected limb.
