摘要
目的比较经典套入式胰肠吻合、套入捆绑式胰肠吻合的临床效果。方法收集行胰十二指肠切除术的患者257例,其中采用经典套入式胰肠吻合方式142例,套入捆绑式胰肠吻合方式115例。分析患者胰肠吻合时间、术后并发症发生率及病死率。结果套入捆绑式胰肠吻合术式较经典套入式胰肠吻合术式胰肠吻合操作时间短,胰漏发生率低(P<0.05)。其余术后并发症各指标、病死率差异无统计学意义(P>0.05)。结论 2种方法均有良好的临床效果及较低的并发症。套入捆绑式胰肠吻合具有操作方便、操作时间短、容易掌握、胰漏发生率较低的优点。
Objective To compare the efficacy of traditional invaginated and binding invaginated pancreaticojejunostomy. Methods The clinical data of 257 patients who underwent pancreaticoduodenectomy from October 2002 to March 2007 were collected, which included 142 patients who underwent traditional invaginated pancreaticojejunostomy and 115 patients underwent invaginated binding pancreaticojejunostomy. The operation time, postoperative complications and death rate were analysed. Results The invaginated binding pancreaticojejunostomy had shorter operative time and less pancreatic leakage than the traditional invaginated pancreaticojejunostomy. There were no significant differences in other complications rate and mortality rate in two groups. Conclusion Satisfactory efficacy and low complication rates can be achieved using any of the two methods of pancreaticojejunostomy. Invaginated binding pancreaticojejunostomy has more convenient procedure, shorter operative time and less pancreatic leakage than the other.
