摘要 目的:寻找全膝关节置换手术股内侧肌间入路的安全区,为减少手术入路的损伤提供局部解剖形态学基础。方法:对10具(20侧)成人下肢尸体标本的股内侧肌血管神经及其毗邻血管神经进行解剖与观测。结果:以髌骨内上缘为原点,支配股内侧肌最远端血管分支X轴范围为35.2~72.3mm,Y轴范围为41.4~78.9mm。最远端神经肌支X轴范围为33.4~77.8mm,Y轴范围为43.4~89.3mm。髌骨内上缘距后方的胴动脉神经血管束平均距离为86mm±13.8mm,范围值为55.2~118.6mm。结论:股内侧肌存在一个33mm×41mm的矩形“乏神经血管区”,在这个区域进行手术操作可减少损伤。 To provide a safe zone for total knee arthroplasty based on the morphometric study. Methods: The nerves and blood vessels for the vastus medialis and adjacent tissues in adult lower limb specimens were dissected and measured. The supply regions of these nerves and blood vessels were identified by introducing a coordinate, in which Y axis was identified as a line paralleled to long axis of the femur through the origin that was midpoint at the superomedial margin of the patella of a line connecting the upper pole and medial point of the patella and X axis was vertical to Y axis through the origin Results: The most distal femoral artery branched to obliquely supply the vastus medialis, with varied ranges from 35.2-72.3mm in X axis and 41.4-78.9mm in Y axis. Additionally, the most distal femoral nerve extensively branched to a region varied from 33.4-77.8mm in X axis and 43. 4-89.3mm in Y axis. The average distance between the patella margin and the popliteal vessels was 86.0±13.8 mm (55. 2 mm in minimum and 118. 6 mm in maximum). Conclusion: These findings suggest that the vastus medialis occupies a 33 mm×41 mm rectangular lack of neurovascular zone, which is recommended as an operation approach.
