摘要
目的 探讨无典型临床表现阑尾炎的ＣＴ诊断价值。方法 搜集临床首次诊断未考虑为阑尾炎，手术和病理证实为阑尾炎者２０ 例，回顾性分析其ＣＴ表现特点。结果 ＣＴ表现有：（１）７ 例表现为阑尾增粗、壁增厚并有明显强化，其中４ 例阑尾内见结石。（２） 盲肠和（ 或） 周围炎１４ 例。（３）右下腹局限性脓肿１１ 例，其中２ 例伴斑点钙化。ＣＴ误诊为回盲部及升结肠肿瘤２ 例，盆腔炎２ 例。结论 症状和体征不典型的阑尾炎临床诊断困难，常误诊，绝大多数病例根据其ＣＴ表现可获得正确诊断。
Objective To investigatethe value of CTin diagnosis of appendicitis with atypical clinical features- Methods CTmanifestationsof20 cases ofappendicitis,which were notinitially considered on clinical presetation,confirmed surgically and pathologically were retrospectively analyzed- Results The CTfindings of appendicitisincluded: (1) The appendix enlarged in diameter, with wall thickening and enhancement after administration ofIVcontrast material(7 cases),presenceofappendicolithin 4 cases- (2) pericecalinflammation (14 cases)-(3) Localized abscessofrightlowerquadrant(11 cases),calcified appendicolithseenin 2 cases- CT misdiagnosed 2 cases as tumour of ascending colon, and another 2 cases as pelvic inflammatory disease- Conclusions The clinicaldiagnosis ofappendicitisisvery difficult when patientspresent with atypicalsignsand symptoms,butin mostcases,the correctdiagnosisofappendicitiscould be made onthe basisofCTfindings-
