原发性高血压胰岛素抵抗与胰岛素受体的关系

摘要 目的 探讨原发性高血压( E H) 胰岛素抵抗与胰岛素受体( In R) 的关系。方法 将65 例新诊断 E H 患者分为30 ~39 岁,40 ~49 岁,50 ~59 岁以及60 岁以上4 个年龄组,与正常人对照,测定胰岛素敏感指数( I S I) 及红细胞 In R,并分析 I S I与 In R 的关系。结果  E H 患者 I S I各年龄组无显著性差异, In R 数目30 ~40 ~50 ~年龄组无显著性差异,60 岁以上年龄组高于30 ~40 ~年龄组( P< 0 .05) ;正常人随着年龄增大 I S I降低,60 岁以上年龄组 I S I低于30 ~40 ~年龄组( P< 0 .05) , In R 数目低于30 ~40 ~50 ~年龄组( P分别< 0 .01 ,< 0 .05 ,< 0 .05) ; E H60 岁以上年龄组 I S I 及 In R 数目与同年龄组正常人无显著性差异,其它 E H 年龄组 I S I及 In R 数目均低于同年龄组正常对照。结论 正常人随着年龄增大,胰岛素敏感性降低,胰岛素抵抗程度增加,胰岛素受体减少; E H 的胰岛素抵抗程度与发病年龄基本无关; In R 缺陷参与 E H 胰岛素抵抗的形成, E H 的 In R 缺陷与 E H 发病年龄密切相关,发病年龄? Objective It deals with the relationships between insulin resistance and insulin recepter (InR) with essential hypertension (EH).Methods 65 newly-diagnosed EH patients are divided into four groups according to their ages:30~39,40~49,50~59 and those over 60.As compared with normal people,insulin sensitive index (ISI) and erythrocyte InR are tested and the relationships between ISI and InR are analysed.Results There is no distinct difference among all the four EH groups in ISI,so is the case with the three former(30~,40~,50~)EH groups in their InR numbers.The InR number of the EH group over 60 is higher than that of the two former groups(30~,40~), P <0.05, ISI of normal people reduces with the increase of age,ISI of the normal control over 60 is lower than that of the two former groups(30~,40~), P <0.05,the InR number of the normal control over 60 is lower than that of the three former normal controls(30~,40~,50~) P is in turn <0.01,<0.05,<0.05;there is no distinct difference between the fourth EH group and the normal control of the same age in ISI and InR numbers.ISI and InR numbers of other three EH groups is lower than that of the normal controls of the same age.Conclusion For a normal person,with the increase of his age the insulin resistance intensifies and the InR number reduces;EH insulin resistance is out of redical relation to the diseasing age;InR defect takes part in the forming of EH insulin resistance,and EH InR defect has a close relation to the EH diseasing age.The diseasing age is younger,the InR defect is more serious.
