摘要
目的 对２４ 例喉咽癌放射治疗作出临床分析。方法 本院自１９７５ 年至１９９０ 年共收治２４ 例喉咽癌，所有病例均为Ⅱ－ Ⅳ期鳞癌。均行单纯放射治疗。结果 生存３ 年以上的病例，原发灶总剂量均为６５Ｇｙ－ ７０Ｇｙ；总剂量小于６５Ｇｙ 和大于７０Ｇｙ 的１４ 例患者均于１ 年左右死亡。结论 说明６５Ｇｙ－ ７０Ｇｙ 是喉咽癌单纯放疗的适宜根治剂量；总剂量超过７０Ｇｙ ，不会提高局控率，反会缩短生存期。对于Ⅲ、Ⅳ期病例，尤其是伴有颈淋巴结转移者，单纯放疗的５ 年生存率仅１４ ％ ，明显比文献报道的综合治疗低，因此Ⅲ、Ⅳ期喉咽癌宜采用手术加放疗的综合治疗。
Twenty-four patients with hypopharyngeal carcinoma were treated by radiation alone between 1975 and 1990. There were 21 males and 3 females, ranging in age from 38 to 76 and the average age were 59. The patients were all examined by direct laryngoscopy and all tumors of the hypopharynx were squarmous cell carcinomas. According to the TNM classification of 1978s UICC, 7 cases were belonged stage Ⅱ,5 cases stage Ⅲ,12 cases stage Ⅳ.70.8 percent of the patients were stage Ⅲ or stage Ⅳ.14 patients presence of cervical lymph node metastases.21 patients received total tumor doses of 62 Gy to 70Gy during 7 weeks. After the full course irradiation, 11 patients also presence of cervical 1 ymph node metastases. The patients receiving total tumor doses of 65Gy to 70Gy showed the best results. When dose was over 70Gy,the primary tumor was not controlled effectively and the survival rate were decreased. We can conclude according to this analysis that surgical resection with adjutant preoperative or post operative irradiation are better for the patients belonged to stage Ⅲ and stage Ⅳ.
