摘要
目的探讨腹腔镜辅助Parks手术治疗低位直肠癌的临床效果及适应证。方法采用腹腔镜辅助Parks手术治疗20例低位直肠癌患者,就手术安全性、手术效果及适应证等方面进行分析。结果全组20例手术均获成功,肿瘤上切缘至肿瘤平均距离为(13.6±3.0)cm,下切缘至肿瘤平均距离为(3.5±1.3)cm,术中、术后病理切片显示两切缘均为阴性,平均手术时间(238.6±45.2)min,术中失血量(307.5±53.6)mL,胃肠功能恢复时间(55.8±12.4)h,术后住院时间(11.5±3.9)d,术后吻合口出血及吻合口漏各1例,吻合口狭窄2例,便频15例,均经非手术治疗治愈,全组无手术死亡。结论只要严格掌握适应证,腹腔镜辅助Parks手术治疗低位直肠癌安全可行,具有微创优势,可在根治切除的前提下获得保肛。
[Objective] To explore the clinical outcome and indication of laparoscopy-assisted Parks operation for low rectal carcinoma.[Methods] Laparoscopy-assisted Parks operation was applied in the treatment of low rectal carcinoma in 20 cases.Subsequently,a retrospective analysis was performed of operative safety,effect and indication.[Results] Laparoscopy-assisted Parks operation was successfully performed in 20 cases without operative mortality.The mean distances from superior and inferior incisal margin to the tumor was（13.6±3.0） cm,（3.5±1.3） cm,respectively.Intra-and post-operative pathological sections of the two incisal margins were negative.The average operative duration was（238.6±45.2） min.The mean intraoperative blood loss was（307.5±53.6） mL.The average enterocinesia recovery time was（55.8±12.4） h and the average postoperative length of stay was（11.5±3.9） d.All the complications including stomal bleeding in 1 case and stomal leak in 1,stomal stenosis in 2 and frequent defecation in 15 were cured by nonoperative therapy.[Conclusion] On the premise of the strictly-controlled indication,laparoscopy-assisted Parks operation can be effectively applied in the treatment of low rectal carcinoma with advantages such as safety and feasibility,distinguished mini-traumatic effect,anus-preserving effect as well as radical resection.
