伤椎置钉与跨节段椎弓根螺钉固定治疗胸腰椎骨折的对比研究 被引量:10

摘要 目的:探讨后路经伤椎置钉与跨节段椎弓根螺钉固定治疗胸腰椎骨折的临床疗效。方法:对88例患者伤椎前缘高度及矢状面Cobb角变化、神经功能恢复、内固定失败率(断钉、断棒及退钉等)及腰背痛等指标进行统计学分析。结果:所有患者平均随访时间18.8(12~40)个月。两组患者伤椎前缘高度、矢状面Cobb角术前、术后1周比较,差异均有统计学意义(P<0.05),但两组间比较,差异均无统计学意义(P>0.05);术后1年与术后1周伤椎前缘高度、矢状面Cobb角比较,A组术后1年时差于术后1周,差异有统计学意义(P<0.05),B组差异无统计学意义(P>0.05);术后1年时两组间比较,B组优于A组,差异有统计学意义(P<0.05)。两组患者神经功能评定比较差异无统计学意义(P>0.05),但B组患者腰背疼痛及内固定失败发生率均较A组低,差异有统计学意义(P<0.05)。结论:经伤椎椎弓根螺钉固定治疗胸腰椎爆裂骨折可以增强内固定系统的牢固性,降低术后矫正角度的丢失及内固定的失败率,是治疗胸腰椎爆裂性骨折的有效方法。
