摘要
目的 探讨乳癌根治术后加速创面愈合的方法，使伴有皮瓣坏死的根治术后乳腺癌病人尽早开始放疗和化疗。方法 用换药法及点状表皮皮片移植、中厚皮片移植和随机皮瓣推移法处理皮瓣坏死创面。结果 使用植皮法创面愈合时间明显短于换药法，网状随机皮瓣优于其他几种植皮法。结论 乳癌根治术后病人若出现中度以上皮瓣坏死应尽早切痂植皮，以随机皮瓣推移植皮术效果最佳。若能制成网状皮瓣可扩大皮瓣面积，效果良好。
Objective To choose an optim al treatm ent for patients w ith flap necrosis after m astocarcinom aradical operation in order to give an earlier radiotherapy and chem otherapy. M ethods To m ake a com para tive study of the effect of different treatm ents on the w ounds of flap necrosis w ith dressing,punctate epi derm al flap skin grafting,m oderate flap skin grafting and reticular random flap skin grafting. Results Thew ound healing tim e w ith skin grafting w as obviously shorter than that w ith dressing. Reticular random flapskin grafting w as superior to other tw o skin grafting m ethods and w as also friction stable. Conclusion Pa tients w ith over m oderate flap necrosis after m astocarcinom a radicaloperation shoud be treated w ith earlyrem oval of crust and follow ed instantly w ith reticular random flap skin grafting,so as to shorten the heal ing tim e of the w ound and to give an earlier radiotherapy and chem otherapy possible.
