摘要
目的探讨高龄初产妇妊娠的分娩方式及其对母婴的影响。方法选择160例高龄初产妇作为观察组,另选同期分娩的适龄产妇160例作为对照组,比较两组孕产妇分娩方式、妊娠合并症、分娩并发症及新生儿情况。结果观察组顺产率低于对照组,剖宫产率高于对照组,差异均显著性(P<0.01)。观察组妊高征、糖尿病、前置胎盘、早产、胎儿宫内窘迫、产后出血、产程异常、巨大儿、低出生体质量儿及新生儿窒息发生率均明显高于对照组(P<0.01或0.05)。结论高龄初产妇的妊娠合并症与并发症明显增多,剖宫产率增高,发生新生儿异常增加,应加强高龄初产妇的围生期保健和产前检查,确保母婴安全。
Objective To explore the childbirth status of elderly primiparous women and its influence on safty of mother and infant.Methods 160 cases of elderly primiparous women were retrospectively analyzed as observation group,and 160 cases of young primiparous women were selected and served as control group during the same period.Delivery modes,pregnancy complications and neonatal situation were compared between the two groups.Results Rate of eutocia in observation group was lower than that of control group（P〈0.01）.However rate of cesarean section were significantly increased in observation group compared with control group（P〈0.01）.Occurrence probability of pregnancy-induced hypertension,diabetes,lead the placenta,preterm,fetal distress,postpartum hemorrhage,labor abnormalities,giant neonate,low birth weight neonate and asphyxia of newborn in observation group was higher than that of control group（P〈0.01 or 0.05）.Conclusion The incidence of complications,cesarean section and neonatal abnormalities was markedly increased in elderly primiparous women.Thus the prenatal and perinatal healths for elderly primipara should be strengthened to ensure the safety of mothers and their babies.
