摘要
目的:以关节镜为标准,比较多序列MRI对髌股关节软骨退变的诊断价值。材料与方法:48个膝关节行关节镜检查及3D-Volume-FFE-T1WI矢状位、SE-T1WI横断位、FFE-T1WI横断位扫描。3D-Volume-FFE-T1WI矢状位行多平面重建(MPR)处理。10例无症状志愿者仅行MRI扫描。MRI图像每个髌股关节分内、外侧髌骨面、股骨滑车面与关节镜图像逐一对照分析。统计各序列诊断软骨病变的敏感性、特异性。结果:关节镜下发现102个病变软骨面,3D-Volume-T1WI敏感性、特异性高于SE-T1WI(P<0.01)。3D-Volume-FFE-T1WI对软骨病变的分级和关节镜分级间有显著正相关性(P<0.01)。结论:3D-Volume-FFE-T1WI结合MPR评价髌股关节软骨退变明显优于SE-T1WI、FFE-T1WI。关节镜和MRI评价关节软骨有很好的相关性、互补性。
Objective: With arthroscopy as the Standard of reference, to evaluate multiple MR sequences imaging in diagnosing of patellofemoralarticular cartilage degeneration. Materials and Methods: Sagittal 3D-Volume-FFE-TlWI, axial SE-T1WI, axial FFE-T1WI and arthroscopy were Per-formed in 48 knee joints. Twenty normal knee joints of healthy Volunteers underwent MRI only. Sagittal images were reconstructed with MPR. Thetrochlear facets, media and lateral patellar facets were compared one by one with arthroscopic pictures. Statistical significant differences in Sensitivityand Specificity were calculated. Results: 1O2 cartilage defects were demonstrated by arthroscopy. The sensitivity and Specificity of 3D-Volume-FFET1WI and conventional FFE-T1WI were significantly higher than that of Standard SE-T1WI (P< 0. 01 ). Grading of cartilage degeneration by 3D-Vo-lume-FEE-T1WI showed parallel relationship with the grading by arthroscoPy (P< 0. 01). Conclusion: Chupled with MPR, 3D-Volume-FFE-T1WIis much superior to SE-T1WI, FFE-T1WI in evaluating articular cartilage degeneration. MRI and arthroscopy are well complementary to each other.
