BRAF^(V600E)基因突变及p27蛋白表达与甲状腺乳头状癌颈淋巴结转移的关系 被引量:5

The Correlation between BRAF^(V600E) Gene Mutation and p27 Expression and Lymph Node Metastasis of Papillary Thyroid Carcinoma

摘要 目的:研究甲状腺乳头状癌(PTC)中BRAFV600E基因突变及p27蛋白表达并评估在PTC颈淋巴结转移危险因素中的意义。方法:采用PCR-SSCP、DNA序列分析、免疫组化等技术,检测83例PTC标本中BRAFV600E基因突变和p27蛋白染色状况。结果:单因素结果提示PTC颈淋巴结转移情况在年龄、临床分期、BRAFV600E基因突变和p27蛋白表达的差异上有统计学意义(P<0.05)。而在性别、原发肿瘤直径和病灶单双叶的差异则无明显意义(P>0.05)。多因素分析结果提示年龄(P=0.009)、BRAFV600E基因突变(P=0.031)和p27蛋白表达(P<0.001)作为评估甲状腺乳头状癌颈淋巴结转移危险因素上的差异有显著性。结论:BRAFV600E基因突变与甲状腺乳头状癌的发生发展密切相关,BRAFV600E基因突变及p27蛋白低表达可以作为评估PTC颈淋巴结转移的危险因素。 Objective:To investigate the BRAFV600E gene mutation and the expression of p27 in papillary thyroid carcinoma,and evaluate their contribution to the lymph node metastasis of PTC.Method:The tissue samples from 83 patients were analyzed for BRAFV600E mutation by PCR-SSCP,gene sequence analysis and for p27 expression by immunohistochemistry.Result:Univariate analysis showed that age,extrathyroidal extension,clininal stages,BRAFV600E mutation and p27 were significant predictors of lymph node metastasis of PTC(P〈0.05),whereas sex,size,location were not.Multivariate analysis confirmed that age,BRAFV600E mutation and low expression of p27(P〈0.005) were strong independent predictors of lymph node metastases in PTC.Conclusion:BRAFV600E mutation was correlated with PTC progression,gene sequence analysis of BRAFV600E and immunohistochemical analysis of p27 expression may be a valuable tool for identifying risk of lymph node metastases.
