摘要
目的探讨围绝经期妇女情绪障碍对其社会功能的影响。方法随机选择2008年6月至2009年3月在江苏省人民医院心理科或者妇科门诊就诊的40～60岁妇女203例,平均年龄(48.13±4.91)岁,进行问卷调查。问卷内容包括一般资料、改良Kupperman绝经指数量表(KMI)、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)、症状自评量表(SCL-90)、生活质量综合评定问卷(GQOLI-74)中的社会功能维度量表的评估。并根据诊断标准将人群分为4组:对照组、围绝经期综合征组(A组)、情绪障碍组(B组)、围绝经期综合征合并情绪障碍组(C组)。对调查数据进行单因素方差分析、相关分析、多元逐步回归分析。结果 B组和C组的社会功能各个因子的评分值均显著低于对照组(P<0.05),而工作学习和业余爱好2个因子的评分值显著低于A组(P<0.05)。A组与对照组之间在社会支持因子评分存在显著性差异[(13.47±2.88)分比(15.38±2.16)分,P<0.05]。围绝经期症状中眩晕、疲乏、性生活改变、情绪波动、抑郁疑心、失眠与社会支持因子呈负相关,其中抑郁与社会支持相关最为明显(r=-0.452,P<0.001)。逐步回归结果表明情绪障碍中抑郁、强迫症状、睡眠障碍是影响社会功能的主要因素。结论围绝经期的情绪障碍可导致社会功能明显降低。
Objective To explore the effects of the mood disturbance on social function in perimenopausal women.Methods This was a cross-sectional study of 203 women aged 40 to 60 years（average 48.13±4.91 years old）from Psychology and Gynecology Out-patient Clinic in Jiangsu Province People＇s Hospital.All subjects were assessed using KMI,HAMA,HAMD,SCL-90,social function domain of GQOLI-74,and were divided into 4 groups：the normal group（n=44）,the perimenopause syndrome group（Group A,n=33）,the mood disturbance group（Group B,n=52）,and the perimenopause syndrome with mood disturbance group（Group C,n=74）.Data were evaluated with ANOVA,correlation analysis and logistic regression.Results Group B and group C got lower scores of social function than that in normal group（P0.05）,and lower scores of work and study,avocation domain of social function than that in Group A（P0.05）.There was significant difference in the social support domain between Group A and the normal group（13.47±2.88 vs 15.38±2.16,P0.05）.No significant difference was found in other domain of social function between Group A and the normal group.Significant correlations were observed between social support and depression（r=-0.452,P0.001）,and other symptoms of perimenopause syndrome（dizzy,decreased libido,sexual dysfunction,emotion fluctuation,insomnia）.Multiple stepwise regression analysis showed that depression,obsession-compulsion,insomnia were major determinants of the scores of social function.Conclusions Mood disturbance might result in lower social function in perimenopausal women.
