摘要
目的探讨胃肠道间质瘤(GIST)的诊断和外科治疗。方法对周口市中心医院2002年至2009年诊断明确的30例GIST患者的临床表现、辅助检查、手术治疗、病理结果及预后进行回顾性分析。结果本组病例酪氨酸激酶受体CD117阳性者29例,阳性率为96.67%;CD34阳性者21例,阳性率为70%,CD117、CD34均阳性者为21例,阳性率为70%。治疗以手术为主,全组无围手术期死亡,未出现严重并发症,均治愈或好转出院。30例患者中随访24例,随访率80%,随访时间6个月～7年。结论 GIST缺乏特征性的临床表现,术前确诊率较低,免疫组织化学检测对确诊本病至关重要,外科手术切除是目前最主要的治疗方法。
Objective To investigate clinical characteristics,diagnosis and surgical treatment of gastrointestinal stromal tumors（GIST）.Methods The clinical pathological and follow-up data of all the 30 patients with GIST admitted in our hospital between 2002 and 2009 were analyzed retrospectively.Results The immunohistochemistry results revealed that the positive rate of CD117,CD34 was 96.67%（29/30）,70%（21/30）respectively.And the positive rate of CD117 and CD34 was 70%（21/30）.The postoperative follow-up period ranged from 6 to 84 months,6 cases were lost（20%）.All the cases underwent successfully surgical procedures,with no operative complications and deaths.Conclusions There is low preoperative diagnosis rate in GIST for their nonspecific digestive symptoms.Pathological and immunohistochemical techniques are essential to the diagnosis of GIST.Radical surgical excision is the main therapy for primary gastrointestinal stromal tumor.
