应用快速原型技术指导腓骨平行折叠重建下颌骨的探讨 被引量:2

The use of rapid prototyping in mandible reconstruction with double barrel fibular graft

摘要 目的:研究如何将快速原型技术与腓骨平行折叠技术相结合,以更好地指导下颌骨重建。方法:下颌骨肿瘤切除术后复杂缺损重建患者6例,术前计算机辅助模拟手术,应用快速原型技术制作腓骨平行折叠后的颌骨模型并预制钛板形态,参照模型,指导术中腓骨骨段精确摆放位置。结果:所有患者创口一期愈合,结合放射核素扫描及数字全景片检查,证实移植腓骨与剩余颌骨完全愈合,腓骨位置与术前设计吻合,患者术后咬合关系良好,面形恢复满意。结论:利用快速成型技术指导腓骨平行折叠,能够更精确地重建下颌骨的外形,有利于保持咬合关系和髁突位置,为二期种植义齿修复奠定基础。 PURPOSE: To evaluate the usefulness of rapid prototyping technique and double barrel fibular graft technique in mandible reconstruction.METHODS: Six patients underwent reconstruction of large segmental mandibular defects with double barrel fibular osteocutaneous graft techniques.3-dimensional mandibular models were made by rapid prototyping technique to ensure that the fibular bones were placed accurately and matched with the remaining mandible segment.RESULTS: Of all patients,the fibular osteocutaneous grafts survived completely,there was no evidence of recurrence.Good appearance and occlusion were achieved in all sufferers after treatment.Panoramic radiograph and three-dimensional CT scan revealed that the locations and heights of the fibular bones met the preoperative design.CONCLUSION: The use of rapid prototyping technique can improve the accuracy of double barrel fibular location in mandible reconstruction,maintain the function of temporomandibular joint and regenerate a symmetrical facial contours.
