摘要
目的掌握洪雅县克山病病情,为克山病防治提供科学依据。方法按照克山病病情监测方案的要求,根据历史病情,选择心肌病发病较高的2个镇中的2个病村作为监测点,分层随机抽查,详细询问病史,体检,描记心电图,对疑似克山病例拍摄后前位X线胸片,按《克山病诊断标准》(GB17021-1997)诊断。检测监测点粮食及发硒含量。结果在病区14个医疗机构搜索心肌病病例259例,未发现克山病。2个监测点心电图描记859人,异常率22.70%(195例);拍摄X线胸片19例,检出慢型克山病2人,潜在型克山病1人,总检出率为0.35%。居民发硒范围0.11~0.55 mg/kg,均值0.40 mg/kg。粮食硒含量范围0.01~0.19mg/kg,均值为0.09mg/kg。结论四川省洪雅县克山病处于低发状态,但是粮食硒含量低,应进行针对性防治。
Objective To study the current incidence of Keshan disease(KD) in Hongya county in Sichuan, andprovide scientific basis for KD prevention and control.Methods Based on the Scheme of KD Surveillance, 2 villages in 2 towns were chosen as surveillance sites by the historical date and higher incidence of cardiomyopathy. Stratified random sampling, detailed history taking, clinical examination and ECG tracings were conducted, posteroanterior X-ray was taken among suspected cases of KD. KD cases were diagnosed according to the Diangnostic Criteria for Keshan Disease (GB 17021 -1997) . Hair selenium level and Food selenium level in Surveillance sites were surveyed.ResultsNo suspected KD was found among 259 investigated cardiomyopathy cases in 14 hospitals. There were 859 residents in two surveillance sites were collected. Electrocardiograms examined 859 residents, and 22.70% (195/859) showed abnormal. Nineteen people were checked by chestX-ray. Two chronic KD cases and one latent KD case were diagnosed. The total detection rate was 0.35%(3/859). The hire selenium level in residents was 0.11 -0.55mg/kg, with mean 0.40mg/kg.The food selenium level was0.01 -0.19mg/kg, with mean0.09mg/kg.ConclusionThe incidence of KD is lowin Hongya County in Sichuan. Lowfood selenium level has inpact on the incidence of KD. The targeted prevention and control measures should be adopted.
