性早熟女童血清胰岛素样生长因子Ⅰ检测价值及抗早颗粒剂治疗的作用 被引量:3

摘要 目的 研究特发性中枢性性早熟(ICPP)女孩血清胰岛素样生长因子Ⅰ(IGF-Ⅰ)及其结合蛋白Ⅲ(IGFBP-3)的检测价值.方法 ICPP组39例和单纯性乳房发育(SPT组)30例初诊时清晨空腹采静咏血,采用化学发光法检测雌二醇(E2)、促黄体素(LH)、卵泡刺激素(FSH)、IGF-1、IGFBP-3,比较两组结果,同时ICPP组口服中药抗早颗粒剂,疗程6个月,治疗前后检测IGF-1、IGFBP-3的变化.结果 ICPP组血清E2、LH、FSH水平均明显高于SPT组(P<0.01或P<0.05);ICPP组血清IGF-Ⅰ、IGFBP-3水平分别为SPT组的1.9和1.4倍;IGF-Ⅰ/IGFBP-3比值为对照组的1.3倍;血清IGF-Ⅰ水平与E2水平呈正相关(r=0.46,P<0.01);抗早颗粒剂治疗6个月后,血清E2浓度较治疗前有显著降低(P<0.01),血IGF-Ⅰ浓度较治疗前无显著变化,GFBP-3浓度较治疗前有增加趋势,但差异无统计学意义.结论 血清IGF-1及IGFBP-3水平检测可作为ICPP早期诊断的参考指标之一,中药对其无显著改变.
