摘要
目的：探讨股骨颈骨折合并同侧股骨干骨折的临床特点及外科治疗的疗效.方法：回顾性分析2005年1月至2011年1月间采用外科治疗的40例股骨颈骨折合并同侧股骨干骨折患者的临床资料.其中11例股骨颈骨折漏诊.全部病例都采取切开复位内固定治疗,其中发现股骨颈骨折的采用空心钉固定的20例; 股骨颈骨折及股骨干骨折同时应用重建髓内钉固定5例;动力髋螺钉固定4例;股骨干骨折行顺行髓内钉固定20例,逆行髓内钉固定9例,股骨干骨折采用动力髋钢板固定11例.结果：治疗随访1.5-3.5年,平均为2.4年;股骨颈漏诊病例中有2例因发现较晚放弃治疗而发生股骨头坏死,有2例发生股骨干延迟愈合;早期发现股骨颈治疗组中有3例出现股骨干骨折延迟愈合;所有病例中有5例出现不同程度的髋膝功能障碍;其余病例愈后良好.结论：股骨颈骨折合并同侧股骨干骨折病例易发生股骨颈骨折漏诊,及时发现有利于股骨颈及股骨干骨折愈合.

Conclusion; treatment follow--up 1.5 years -- 3.5 years, average 2.4 years; Femoral neck misdiagnosis cases have 2 patients with found late forgo treatment occurred in 2 cases, avascular necrosis of the delay hap- pened femur heal; Early detection of femoral neck treatment groups had 3 cases femoral fractures delay healing; All cases have 5 cases with different degrees of hip knee function obstacle; The rest of the cases after more good. Conclusion femoral neck fracture with lateral femoral fractures merger case easy happening femoral neck fracture misdiagnosis, the timely discovery for femoral neck and femoral fractures healed.
