糖皮质激素对儿童重症肌无力Th1/Treg细胞转录因子表达的影响及机制研究 被引量:7

摘要 目的:探讨糖皮质激素治疗对儿童重症肌无力(Myasthenia gravis,MG)Th1/Treg细胞转录因子表达及相关细胞因子的影响。方法:应用RT-PCR技术检测52例MG患儿和20例健康对照者外周血单个核细胞(Peripheral blood mononuclear cell,PBMC)T-bet和FOXP3 mRNA的表达情况,应用ELISA法检测相关细胞因子干扰素-γ(Interferon-gammaI,FN-γ)、白细胞介素-6(Interleukin-6I,L-6)及转化生长因子-β1(Transforming growth factor-β1,TGF-β1)的浓度变化情况。结果:与健康对照组比较,患儿T-bet mRNA的表达显著增高(P<0.05),而激素治疗前组与治疗后组比较无显著性差异。激素治疗前组FOXP3mRNA的表达显著高于健康对照组(P<0.05),激素治疗后组与健康对照组比较,差异无统计学意义,激素治疗前后组无显著性差异。T-bet mRNA与FOXP3 mRNA的表达比值,激素治疗前组显著高于健康对照组(P<0.05)。激素治疗后组与健康对照组及治疗前组比较I,FN-γ的浓度显著升高(P<0.05),而健康对照组与治疗前组比较无显著性差异。3组之间I,L-6和TGF-β1的浓度无显著性差异。结论:T-bet mRNA和FOXP3 mRNA在MG患儿中表达异常,表明Th1和Treg细胞参与了MG的发病,经激素治疗后免疫功能有所恢复;本组患儿IL-6和TGF-β1的浓度无明显异常,提示这些患儿免疫功能紊乱程度较轻,未达到显著差异水平;而在一定情况下I,FN-γ在治疗后其水平升高可能对机体发挥着一定的保护作用。 Objective: To investigate the effect of glucocorticoid treatment on the expression of Th1/Treg cell transcription factor and related cytokines in children with myasthenia gravis(MG).Methods: RT PCR was used to detect the expressions of T bet and FOXP3 mRNA in children's peripheral blood mononuclear cells of 52 MG patients and 20 healthy children;ELISA was applied to analyze related cytokines IFN γ,IL-6 and TGF-β1 concentration changes.Results:Compared with the healthy control group,the T bet mRNA expression was significantly higher(P0.05) in children with MG,while the groups before and after hormone treatment were of no significant difference.In the group before hormone treatment,FOXP3 mRNA expression was significantly higher than that in the control group(P0.05),while the hormone therapy group and the healthy control group were of no significant difference and the groups before and after hormone therapy were of no significant difference.For the ratio of T-bet mRNA expression to FOXP3 mRNA,the groups before hormone therapy was significantly higher than in healthy controls.In hormone therapy group,compared with the healthy control group and before hormone therapy group,IFN γ concentration was significantly higher(P 0.05),while the healthy control group and the before treatment group showed no significant difference.In the three groups,IL-6 and TGF-β1 levels were not significantly different.Conclusion: T bet mRNA and FOXP3 mRNA expressions are abnormal in children with MG.Th1 and Treg cells are concerned with the pathogenesis of MG.After hormone therapy,the immune function has been restored to some extent.In this research,IL 6 and TGF-β1 levels are not abnormal,suggesting a less extent in children with immune dysfunction and no significant difference;in some cases,IFN γ levels increase after treatment,which suggests IFN γ may also play a role in the protection of the body.
