摘要
目的:了解合理用药软件(PASS)对某院住院患者用药情况的监测作用,分析该医院医嘱合理用药情况,以促进临床合理用药。方法:利用PASS对一周内所有临床科室医嘱情况进行监测,分别统计问题医嘱发生率、类型、修改情况及具体药物。结果:在选定时段内监测结果涉及2 421名患者7 387条问题医嘱;按警示级别计,问题医嘱发生率为3 051.22;按监测类型计,问题医嘱主要发生在药物相互作用和剂量审查方面,发生率分别为1 134.42,1 237.92;各类问题医嘱修改率大约为60%;问题医嘱中出现最多的药物依次为螺内酯片、西咪替丁注射液、盐酸法舒地尔注射液、盐酸贝那普利片、维生素C注射液。结论:PASS能较准确监测医院问题医嘱发生情况,促进安全合理用药,对减少临床不合理用药有较高的利用价值。
Objective： To understand, monitor and analyze the clinical medical orders by prescription automatic screening system （PASS） in order to promote rational drug use. Method： PASS was used to monitor all clinical medical orders in a week, calculate the rate, type and revision of the question medical orders, and analyze the included medicines. Result： The investigated prescriptions involved in 7 387 prescriptions and 2 421 patients. According to the warning level, incidence rate of question orders was 3 051.22. According to the type of question orders, such as drug interaction and dosage, the incidence rate was 1 134. 42 and 1 237. 92,respectively. The revision rate of all the orders was 60%. The drug appeared in the question orders at high frequency was spironolactone tablet, cimetidine injection,fasudil hydrochloride injection, benazepril hydrochloride tablet and vitamin C injection by turns. Conclusion： PASS could be used for monitoring problems in the medical orders accurately, improving rational and safety drug use in hospitals.
