摘要
目的初步探讨儿童原发性肾病综合征(PNS)脂联素(ADPN)的变化,了解ADPN与PNS其他脂代谢相关因素的联系。方法对71例PNS急性期患儿、37例缓解期患儿和35例健康儿童的血浆、尿液ADPN浓度进行定量分析,并将急性期患儿分为激素敏感组59例和激素耐药组12例,测定急性期患儿的体质指数(BMI)、肾小球滤过率(GFR)、血清白蛋白(Alb)、肌酐(Cr)、补体C3、三酰甘油(TG)、胆固醇(Tch)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、脂蛋白a[LP(a)]、载脂蛋白A(apoA)、载脂蛋白B(apoB)以及24 h尿蛋白定量,并进行相关性分析。结果急性组的血浆、尿ADPN(14.02±3.44μg/ml,4.71±2.12μg/ml)显著高于缓解期组(7.13±5.07μg/ml,2.50±1.06μg/ml)和正常对照组(7.17±2.94μg/ml,2.60±1.06μg/ml)(P<0.01)。激素耐药组血浆、尿ADPN均显著高于激素敏感组(P<0.05)。PNS急性期患儿血浆ADPN分别与尿ADPN、血清TG、Tch、LDL、LP(a)、apoB及24 h尿蛋白定量正相关(r=0.242 7~0.609 1,P均<0.05),与血清Alb、HDL负相关(r=-0.745 4、-0.489 7,P<0.01)。结论 PNS患儿的血浆及尿液ADPN浓度明显升高。血浆和尿液ADPN浓度与其他脂代谢指标密切相关。
Objective To investigate the level of adiponectin(ADPN)in children with primary nephrotic syndrome(PNS)and the relationships between plasma ADPN and other lipid metabolism related factors.Methods The levels of plasma and urine ADPN were quantitatively measured in 71 patients with PNS in acute stage(acute stage group)and 37 patients with PNS in complete remission(remission stage group)and 35 healthy children(control group).Patients with PNS in acute stage were divided into steroid-responsive group(159 patients)and steroid-resistant group(12 patients).The body mass index(BMI),glomerular filtration rate(GFR),serum albumin(Alb),creatinine(Cr),complement 3(C3),triglyeride(TG),cholesterol(Tch),high density lipoprotein(HDL),low-density lipoprotein(LDL),lipoprotein(a)(LP(a)),apolipoprotein a(apoA),apolipoprotein b(apoB),and 24-hours proteinuria were measured.The relationships between these measurements and plasma ADPN were simultaneously analyzed.Results The levels of plasma and urine ADPN(14.02 ± 3.44 μg/ml,4.71 ± 2.12 μg/ml)were significantly higher in acute stage group than that in remission stage group(7.13 ± 5.07 μg/ml,2.50 ± 1.06 μg/ml)and control group(7.17 ± 2.94 μg/ml,2.60 ± 1.06 μg/ml)(all P 〈0.01).In acute stage group,the levels of plasma and urine ADPN were significantly higher in steroid-resistant group than that in steroid-responsive group.There were significant positive correlations between plasma ADPN and urine ADPN,TG,Tch,LDL,LP(a),apoB and 24-hour proteinuria(r = 0.242 7-0.609 1,all P〈 0.05),whereas there were significant negative correlations between plasma ADPN and Alb,HDL(r =-0.745 4,-0.489 7,P 〈0.01).Conclusions The levels of plasma and urine ADPN are markedly increased in patients with PNS.The levels of plasma and urine ADPN are closely related with other lipid metabolism related factors.
