摘要
目的比较股骨近端抗旋髓内钉（PFNA）与动力髋螺钉（DHS）治疗股骨转子间骨折的疗效。方法 93例股骨转子间骨折患者按照手术方法分为两组,PFNA组51例,DHS组42例,比较两组的疗效。结果患者均获随访,时间3~15（9.6±3.2）个月,骨折均愈合。手术时间：PFNA组为（60.2±15.4）min,DHS组为（117.8±32.6）min;差异有统计学意义（t=4.521,P〈0.05）。下地活动时间：PFNA组为（3.6±0.6）d,DHS组为（7.1±2.2）d;差异有统计学意义（t=4.769,P〈0.05）。并发症：DHS组螺钉切出股骨颈6例、髋内翻6例,PFNA组1例因术后心功能衰竭死亡,差异有统计学意义（χ2=4.001,P〈0.05）。结论 PFNA操作简便,手术时间短,创伤小,术后并发症少,适用于老年股骨转子间骨折的治疗。
Objective To investigate and compare the proximal femoral nail anti-rotation（PFNA） and dynamic hip screw（DHS） in treatment of intertrochanteric fracture.Methods 93 cases were divided into two groups according to surgical methods.51 cases were treated with PFNA,and 42 cases were treated with DHS.The clinical effect were observed.Results All the patients were followed up for 3~15（9.6±3.2） months,and firm union of fractures were obtained in all the cases.The average operation time was（60.2±15.4）min in PFNA group,shorter than that of（117.8±32.6）min in DHS group（t=4.521,P0.05）;and the first exercise time in PFNA group were shorter than that in DHS group（（3.6±0.6）d vs（7.1±2.2）d,t=4.769,P0.05）.Complications happened in 12 cases in DHS group： 6 cases with screw cutting out femoral neck and 6 cases with coxa vara.While in PFNA group,one case died of postoperative heart failure.There was significant difference in complications between the two groups（χ2=4.001,P0.05）.Conclusions PFNA has the advantages of less surgical trauma,less complications,and shorter operation time.So it is a useful device in the treatment of intertrochanteric fracture in the elderly.
