MR-DWI成像和CT引导穿刺活检对良、恶性肺部病变诊断价值的探讨 被引量:8

Diagnostic value of diffusion-weighted MR imaging and CT-guided biopsy in pulmonary benign and malignant lesion

摘要 目的:探讨MR扩散加权成像(DWI)及表观扩散系数(ADC)值测量对肺良恶性病变的诊断及鉴别诊断价值。材料与方法:2008年4月-2009年12月,经MR-DWI和CT引导穿刺活检组织病理学证实的良、恶性肺部病变52例,其中良性病变17例,肺癌35例。对其DWI信号强度和ADC值进行分析,应用受试者操作特征曲线(ROC曲线)分析评价最佳ADC截断值诊断肺癌与肺良性病变的能力,对其诊断效能与CT进行对照。结果:肺癌与良性病变实性区信号强度和平均ADC值差异有统计学意义fP〈0.05)。肺癌区肿瘤坏死及肺不张、阻塞性肺炎与肺癌肿瘤实性区平均ADC值配对检验差异有统计学意义(P〈0.05)。DWI显示病变内坏死、肿瘤及肺不张优于CT。诊断良、恶性肺结节性病变的最佳ADC截断值为1.24χ10^-3mm^2/s。结论:ADC值是鉴别肺部良、恶性病变的可靠指标;DWI判断肺肿瘤内坏死和周围肺不张、阻塞性肺炎较准确;对确定CT穿刺活检靶点,提高肺内病变活检成功率很有帮助。
