全自动细胞图像分析系统在鉴别良恶性胸腹水中的应用 被引量:12

Differential diagnosis values of benign and malignant pleuroperitoneal fluids by automatic imaging cytometer

摘要 目的应用全自动细胞图像分析系统(AICM)进行细胞核DNA定量测定在鉴别良、恶性胸腹水中的价值。方法 144例良、恶性胸腹水标本经离心涂片,每例制成2张薄层细胞片,1张细胞片进行HE染色,用于常规细胞学检测;另1张Feulgen-Thionin染色,用全自动细胞图像分析系统进行细胞核DNA定量测定检查。结果 144例胸腹水标本经临床或病理诊断证实,91例为良性,53例为恶性。AICM-DNA倍体分析在判断良、恶性胸腹水中的敏感性、特异性、准确性及Youden指数分别为84.9%、100%、93.1%和0.85;而常规细胞学分别为62.3%、79.1%、72.9%和0.41;两者之间灵敏度、特异度、准确度和Youden指数均差异显著(P<0.01)。结论对AICM进行细胞核DNA测定有助于提高胸腹水阳性诊断率,在良、恶性胸腹水的鉴别诊断中较常规细胞学方法更客观、敏感。 Objective To explore the differential diagnosis values of benign and malignant pleuroperitoneal fluids by AICM-DNA ploidy analysis.Methods In 144 samples of pleuroperitoneal fluids,cells were spun on 2 slides by cytospin in each sample.One slide was stained by HE method for cytology analysis.And other slide was stained with Feulgen method for DNA ploidy analysis by using automatic imaging cytometer.Results Clinically or pathologically,of 144 samples of pleural effusion,91 were classified as benign and other 53 as malignant.The sensitivity,specificity,accuracy,and Youden’s index of DNA imaging analysis were 84.9%,100%,93.1%,and 0.85,respectively.However,the sensitivity,specificity,accuracy,and Youden’s index of cytological investigation were 62.3%,79.1%,72.9%,and 0.41,respectively.There were significant difference in the sensitivity,specificity,accuracy and Youden’s index.Conclusion AICM-DNA ploidy analysis is helpful in raising the positive rate of cytopathological diagnosis in malignant pleuroperitoneal fluids,and AICM can be used to differentially diagnose benign and malignant pleuroperitoneal fluids,which is more objective and sensitive than routine cytological methods.
