摘要
目的探讨益血汤对玻璃体切割术后增生型糖尿病性视网膜病变的治疗作用。方法将玻璃体切割术后3天至7天视力不提高,且视网膜无新鲜出血的增生型糖尿病性视网膜病变患者62例(62眼),随机分成中药治疗组(A组,23例23眼),西药对照组(B组,20例20眼)、空白对照组(C组,19例19眼)。A组口服益血汤治疗,B组口服羟苯磺酸钙胶囊;C组为空白对照。3组均予口服多维元素片等,治疗时间为30天。观察治疗前后最佳矫正视力及眼彩色多普勒超声各指标情况。结果视力疗效:A、B、C组的有效率分别为65.2%、45.0%、15.8%,A组结果分别与B、C组比较,差异有统计学意义(P<0.05)。眼彩色多普勒超声检查:A组、B组治疗后眼动脉收缩期峰值血流速度(Psv)、舒张末期血流速度(Edv)、平均血流速度(Vm)较治疗前增高,阻力指数(RI)和搏动指数(PI)较治疗前降低,差异有统计学意义(P<0.01,P<0.05);A组各指标改善情况较B组好,差异有统计学意义(P<0.05)。结论益血汤的运用可以明显改善玻璃体切割术后增生型糖尿病性视网膜病变患眼的眼底供血,增进视力。
OBJECTIVE To investigate the effect of Yixue Decoction on proliferative diabetic retinopathy after vitrectomy surgery.METHODS Selected 62（62 eyes）proliferative diabetic retinopathy cases as the research objects who did not improve visual acuity post-vitrectomy 3-7 days without fresh bleeding in the retina.Randomly divided into traditional Chinese medicine treatment group（group A： 23 eyes of 23 cases）;western medicine control group（group B： 20 eyes of 20 cases）and blank control group（group C： 19 eyes of 19 cases）.Group A was treated with Yixue Decoction,group B with calcium dobesilate capsules while group C as blank control.All groups were given oral treatment with multi-dimensional vitamin tablets for 30 days.Contrasted the index of eye color Doppler ultrasound and best corrected visual acuity before and after treatment.RESULTS Visual acuity improved efficacy： group A was 65.2%,group B was 45.0%.The Visual acuity of group A and B was significantly improved than before the treatment（P0.05）while group C had no significant changes in visual acuity（P0.05）.The index of eye color Doppler ultrasound,the ophthalmic artery peak systolic velocity（PSV）,end diastolic velocity（EDV）and averaged maximum velocity（TAMV）of group A and B all significantly increased;the resistance index（RI）： pulsatility index（PI）of both group A and B decreased.After treatment： hemodynamic parameters were significantly improved（P 0.05）.Statistical analysis of the effect by SPSS software： group A and group B was significantly different（P 0.05）;group A and group C was also significantly different（P0.05）.CONCLUSIONS The treatment of Yixue Decoction on proliferative diabetic retinopathy patients after vitrectomy could improve the retinal blood supply and visual acuity.
