摘要
目的探讨降低肌腱断端回缩张力的方法,提高肌腱缝合质量。方法设计并制作了肌腱减张器,将20根新鲜来亨鸡趾深屈肌腱分两组,将其切断,分别用减张器及不用减张器重新缝合,然后进行力学测试。临床应用于10例共37指Ⅴ区屈肌腱损伤患者(A组)的肌腱缝合手术,同期15例46指(B组)Ⅴ区屈肌腱损伤患者手术未使用减张器,将两组患者术后功能恢复情况进行比较。结果①使用减张器缝合肌腱组力学测试拉力负荷高于比较组。②临床病例A组优:22指,良11指,可4指,优/良总比率为59.5%,B组优17指,良15指,可14指,优/总比率为37.0%,两组比较有显著差异,前者优/总比率高于后者。结论本研究设计制作的肌腱缝合器可有效降低肌腱缝合口张力,提供无创操作条件,提高肌腱缝合的质量。
Objective To explore the methods of relaxing the tension of the tendon,and to improve the quality of tendon suturing.Methods The self-designed tension-relaxing stick were made.20 flexor digitorum profundus muscle tendons from chickens were divided into 2 groups randomly,with 10 in each group.The tendons were sharply transected.10 were repaired by the tension-relaxing stick,and the other 10 not.Then the clinical study was carried in the repair of 37 flexor tendon injury in zone Ⅴ with tension-relaxing stick in 10 cases（A group） and 46 tendon injury repaired with routine way in 15 cases（B group）.Results The tensile strength of the 10 tendons that were repaired with the tension-relaxing stick was higher than the others.And in the clinical study,there were 22 of perfect,11 of good,4 of normal with the perfect rate of 59.5% in A group,and 17 of perfect,15 of good,14 of normal with the perfect rate of 37.0% in B group.There was difference between the two groups of the perfect rate in the statistic analysis.Conclusion The tension-relaxing stick can reduce the tension of the tendon,offer the non-traumatic operating conditions,and improve the quality of the tendon suturation.
