摘要
目的:总结颅内大型脑膜瘤的显微手术治疗经验。方法:回顾分析显微镜直视下手术切除20例颅内大型脑膜瘤的临床资料,其中矢状窦旁和大脑镰脑膜瘤7例,大脑凸面脑膜瘤4例,中颅窝脑膜瘤4例,蝶骨嵴脑膜瘤3例,后颅凹脑膜瘤2例。结果:肿瘤全切除17例,次全切除3例。无严重并发症或死亡病例。术后2个月全部病人症状改善,随访2a脑膜瘤复发1例。结论:选择好手术入路和运用好显微外科技术可以提高大型脑膜瘤的全切率,降低死亡率。
Objective：To evaluate the experience of mierosurgery for intracranial large meningiomas. Methods ：20 eases were analyzed respectively,of which there were 7 parasagittal and parafalx meningioma patients ,4 convexity meningioma patients ,4 middle fossa meningioma patients ,3 sphenoid wing meningioma patients ,2 posterior fossa meningioma patients. Results：Of all the 20 patients, 17 had total resection of tumor, 3 had subtotal resection of tumor. No post - operative death or serious complications occurred. After the operation, all the patients achieved an improvement in clinical symptom. During the follow - up period 2 years, the tumor recurred in 1 patient. Conclusion：Total resection can be achieved in most patieus with microneurosurgieal technique. Proper choice of operative approaches may raise the total removal rate and lower the operative mortality.
