摘要
目的 研究持续强负压吸引在治疗颈乳糜瘘中的作用。 方法 采用YBDS13 型微电脑输液泵、新鲜离体静脉与肌肉及废弃输血袋为材料,进行持续负压吸引治疗颈部乳糜瘘的模拟实验 结果 实验组Ⅰ( 静脉含肌肉者) 的侧支流速随着负压从0 上升至5 kPa 和60 kPa 绝对值时,侧支流速从每3 min 的1.4 ml 升至5.04 ml 和9.30 ml,而负压袋引流量从每3 min 的8.6 ml 减少至4.96 ml 和0.70 ml。 结论 持续强负压吸引不同于一般负压吸引;它不但具有后者的引流作用,还具有后者没有的间接对瘘口加压、堵塞、促进胸导管侧支循环的双重作用。
To study the role of high persistent vacuum drainage in the treatment of cervical chylous fistula.METHODS A simulation experiment on management of cervical chylous fistula was performed with transfusion pump of YBDS 13 microcomputer,a piece of fresh muscle with vein and a set of plastic transfusion equipment.RESULTS In experiment Ⅰ (with muscle and vein) the runoff of the sideway was increased step by step from 1.4, to 5.04 and 9.30 ml/3?min and the amount of drainage in the plastic transfusion bag was decreased step by step from 8.6 to 4.96 and 0.70 ml/3 min along with the change of the absolute value of the negative pressure from 0,5 to 60 kPa.CONCLUSION It is suggested that high persistent vacuum drainage is different from the general closed wound suction. The former not only drains the wound drainage,but also exerts pressure on the wound and the fistula,promotes the establishment of the collecteral circulation of thoracic duct and so results in closure of the fistula.
