摘要
目的评价颏下岛状肌皮瓣修复口腔颊粘膜癌术后组织缺损的临床效果。方法 2007年9月～2009年9月应用面动脉-颏下动脉供血的颏下岛状肌皮瓣修复颊癌术后组织缺损11例,其中男性7例,女性4例,年龄39岁至85岁,平均年龄62岁,病理类型:高分化鳞癌9例,中分化鳞癌2例;T1N0M0 2例,T2N0M0 6例,T3N0M0 3例,皮瓣大小为7cm×4cm～12cm×5cm。结果 11例面动脉-颏下动脉岛状肌皮瓣全部成活,10例患者颊部、颈部创面一期愈合,1例患者术后一周出现口底颌下瘘,经换药处理后颈部创面愈合。患者颊部缺损修复形态良好,患者语音、咀嚼、吞咽功能恢复良好,无面瘫等继发功能障碍,术后无明显张口受限,术后术前对比未见明显颌面部畸形,术后随访1a～2a,颊部及颈部未见肿瘤复发及转移。结论面动脉-颏下动脉颏下岛状肌皮瓣血供明确可靠,制备方便,适宜修复颊癌术后组织缺损。
Objective To explore the clinical application of submental myocutaneous island flap in repairing buccal defects after caner ablition. Methods Clinical data of 11 caes, buccal defects were repaired with facial atery - submental artery island myocutaneous flap, among them, 7 patients were males and 4 were females ; the ages of the patients ranged from 39 to 85 years （62 years on average） , two patients were classified as T1N0M0,six as T2NOM0, three as T3N0M0, the sizes of the flaps ranges from 7 cm ×4 cm to 12 cm × 5 cm. Results The facial atery - sub- mental artery island myocutaneous flap of the all patients survived well, 10 patients of the surgical incision of buccal and neck healed in first phase,including 1 patient with submental fistula after surgical incision one week but healed with after changing dressings, appearance of the buccal in the patients recovered well and speech, chewing as well as swallowing fuctions re - obtained satisfactorily. Without limitation of mouth opening, facial deformity and paralysis or other complications, all patients were followed up 1 to 2 years, and no one showed recurrence or matcstasis in buccal area and in head and neck. Conclusion The facial atery - submental artery island myocutaneous flap have good blood supply and convenient to be prepared, and suitable to repair the buccal tissue defects after the cancer ablition.
