摘要
目的探讨应用参麦注射液及水乐维他辅助治疗浅表性膀胱癌（SBC）术后吡柔比星（THP）膀胱灌注不良反应的效果。方法 96例患者随机分成实验组和对照组,手术后2组均行THP膀胱灌注。实验组除行THP膀胱灌注外,同时给予5%葡萄糖液250 mL加参麦注射液20~40 mL,和5%葡萄糖液250~500 mL加水乐维他10~20 mL,静脉滴注5~10 d。2组定期膀胱镜及尿脱落细胞检查,随访1.5 a观察肿瘤复发和不良反应情况。结果实验组精神健康（MH）得分膀胱灌注前后分别为（82.3±13.4）分和（87.3±11.2）分,生理职能（RP）膀胱灌注前后分别为（71.9±10.4）分和（72.2±21.3）分,躯体疼痛（BP）膀胱灌注前后得分分别为（62.1±28.9）分和（81.4±14.8）分;对照组相应评分分别为（76.9±17.1）分和（71.9±15.1）分,（74.1±12.5）分和（65.5±23.7）分,（60.3±26.9）分和（71.3±15.7）分。2组治疗后各评分比较均有显著性差异（P〈0.05或0.01）。尿路刺激症状发生率实验组为37%,对照组为53%;全身症状发生率实验组为6%,对照组为14%。2组比较均有显著性差异（P均〈0.05）。结论 THP膀胱内保留灌注联合参麦注射液及水乐维他用于浅表性膀胱癌术后,对患者的生存质量有积极意义,患者耐受性好,不良反应少。
Objective It is to approach the influence on the side effect of bladder irrigation with THP after operation of superficial bladder cancer（SBC）,which combined with adjuvant therapy of Shenmai injection and Soluvita.Methods90 patients were randomly divided into experiment group and control group,both groups were treated as THP bladder irrigation.Except for the THP bladder irrigation,experiment group also was treated with Shenmai injection 20 to 40 mL added into 5% glucose liquid 250 mL,and Soluvita 10 to 20 mL added into 5% glucose liquid 250 to 500 mL,the intravenous injection was executed for 5 to 10 days.The condition of tumor reoccur and untoward effect were observed by the detection of cystoscope and urine cast-off cell in 1.5 years.Results In experiment group,the score of mental health were 82.3±13.4 and 87.3±11.2 around the THP irrigation,physiological function score separately were 71.9±10.4 and 72.2±21.3,the score of body pain were 62.1±28.9 and 81.4±14.8;the corresponding score in control group were 76.9±17.1 and 71.9±15.1,74.1±12.5 and 65.5±23.7,60.3±26.9 and 71.3±15.7.There were significant difference in score between both groups（P0.05 or 0.01）.The incidence rate of urinates road stimulation symptom was 37% in experiment group and 53% in control group;the incidence rate of constitutional symptom was 6% in experiment group and 14% in control group.There were significant difference between both groups（P all 0.05）.Conclusion The application of THP bladder irrigation combined with Shenmai injection and Soluvita in postoperation of SBC have positive significance to improve the patients＇ survival quality,and patients have good tolerance and little side effect.
