摘要
[目的]介绍先天性侧旁半椎体切除术的适应年龄和手术方法。[方法]经后路暴露双侧的椎板,先松解对侧的关节突外侧和横突,然后再分离暴露多余的半椎体,用薄刃骨刀和尖刀片切除半椎体后,再用弓根螺钉加钢丝闭合截骨间隙。[结果]本组预防性手术治疗先天性侧旁半椎体畸形56例,经远期随访取得较好的效果,在年龄问题上作者主张早期预防性作多余的半椎体切除,本组病例最早的3岁时作了预防性半椎体切除术。2例术后发生椎弓根螺钉或压缩钢丝断裂,其余病例无其他神经系统并发症发生。[结论]先天性侧旁半椎体切除术,越早进行远期效果越好,且手术简单易行、出血少。15岁以上的患儿进行半椎体切除和矫正侧弯时,则应同时作全脊柱截骨术,方能达到矫正脊柱侧凸的目的。
[Objective]To introduce the operative methods and adaptive age of congenital lateral hemivertebrectomy. [Methods]Bilaterallamina was exposed through posterior approach firstly,adhesive soft tissue in counterlateral articular process and transverse process were relaxed,separated and exposed the surplus hemivertebra.After the hemivertebra was resected,pedicle screw compressive wire was used to close the space of osteotomy.[Results]Fifty-six cases of congenital lateral hemivertebra deformity were treated with operation,with a long term follow-up to get a better effect.In the age issue,the author advocated early resection.A 3 years old patient received preventive resection,who was the youngest one in this group.Complications included pedicle screw or wire rupture in 2 cases,without other nerve system complication.[Conclusion]Early operation leads to better surgical effect,and the operation is simple and with less hemorrhage.When the patient is more than 15 years old,total spinal osteotomy should be carried out at the same time,which is the only method for correcting deformity.
