摘要 目的探讨少白细胞血输血与术后感染的关系。方法检索中国学术期刊全文数据库、维普及万方数据库,收集了1979年至2009年国内公开发表的少白细胞血输血的手术病例对照研究论文,并对文献进行异质性检验。以少白细胞血输血为治疗组及常规输血为对照组,比值比(OR值)为效应指标,然后用Meta分析软件RevMan 4.2进行统计处理。同时给出Meta分析森林图和倒漏斗图。结果最终进入系统评价的文献13篇,其中少白细胞血输血治疗896例,常规输血治疗881例。Meta分析结果合并OR为0.22,95%CI[0.15~0.32],合并效应检验P〈0.001有统计学意义。结论目前相关研究结果的Meta分析显示,少白细胞血输血可以减少术后感染。 Objective To approach the relationship between leukocyte-reduced blood transfusion and postoperative infection.Methods By retrieving full-text database of academic periodical of china,VIP and Wanfang′ Database,domestic overt publication of case-control studies during 1979-2009 on leukocyte-reduced blood transfusion after surgical operation were collected,and a test of heterogenicity of the literature was made.Patients transfused with leukocyte-reduced blood were served as treatment group and patients with conventional blood transfusion as control group,and odds ratio as an effectiveness index.Meta analytic software of RevMan 4.2 was used for statistic process.Forest picture and down-funnel picture were performed.Results At the end,13 articles were judged as qualified and enrolled in the systemic review,with 896 cases of leukocyte-reduced blood transfusion and 881 cases of conventional blood transfusion included.The combined OR of Meta analysis was 0.22,95% CI[0.15-0.32],with statistical significance.Conclusion Meta analysis of present related studies indicates that leukocyte-reduced blood transfusion can reduce postoperative infection.
