摘要
本文探讨26例前交叉韧带胫骨附着部撕脱性骨折的疗效。11例行钢丝内固定术,15例行保守治疗。所有病人在治疗前和治疗后两年,在最大作用力下行Lachman应力位摄片,根据双膝对比得出患膝胫骨前移程度。根据Lysholm评分系统进行评分,手术病人由术前平均62分升高到93分,保守治疗病人由平均72分升高到90分,优良率达92％。在考虑骨折分型的基础上,在应力位片双膝胫骨前移差异>3mm和常规前后位片骨折块向上移位>3mm有手术指征,同时手术亦要考虑患者症状的严重程度、合并症。
Twenty - six patients who had a avulsion fracture of the anterior cruciate ligament adhering on the tibia were reviewed.The operative procedures which consisted of wire fixation of the fragment were done in 11 cases and conservative treatment were done in 15 cases. All patients had measured with stress radiographs of Lachman test between pretreatment and follow-up two years later, the testing parameters compared with knees were the extent of anterior of the injuried knees at maximum manual force. All knees were evaluated with the scale of Lysholm. The mean score of the operative pa-tients increased from 62 points preoperatively to 93 points postoperatively, and conservative patients increased from 72 points to 90 points. The excellent or good rate was 92 % . Measuring with the stress radiographs and comparing with other physical examination, Lachman test has more sensitivity. Considering the classification of the degree of fractured frag-ment displacement, we used a difference between the injuried and the normal knee by 3mm or more at stress radiographs and a superior 3mm or moredisplacement of fragment at anterioposterior radiograph as an operative indication. At the same time, operative treatment must consider the degree of symptoms and complications.
