摘要
目的:探讨EDTA-K_2抗凝剂致血小板假性减少的原因及纠正方法.方法:选取健康体检者30名和EDTA-K_2依赖性血小板假性减少者5名,同时采集EDTA-K_2抗凝静脉血2ml和置于160μl稀释液中的末梢血40μl,分别在sysmex-2000i全自动血球计数仪进行静脉血和末梢血的血小板计数,比较静脉血和末梢血的血小板数,同时用光学显微镜对瑞氏—姬氏染色后的EDTA-K_2抗凝血进行血涂片观察.结果:健康组EDTA-K_2抗凝静脉血和稀释的末梢血血小板数相比,差异不明显,无统计学意义(P>0.05).血涂片上血小板大小、形态、分布正常.EDTA依赖性血小板减少患者静脉血和稀释的末梢血血小板计数相比,差异明显,有统计学意义(P<0.001).在血涂片尾部有大量血小板聚集,数量不等.结论:EDTA-K_2抗凝剂可对部分患者血小板计数造成假性减低,但通过稀释的末梢血计数血小板联合涂片观察,就可以纠正假性血小板减低,避免临床误诊的发生.
Objective:To investigate the reasons and corrective methods of pseudothrombocytopenia induced by anticoagulant EDTA-K_2.Methods:Selection 30 healthy people and 5 EDTA-K_2-dependent pseudothrombocytopenia (PTCP)patients,collected 2ml EDTA-K_2 anticoagulanting venous blood and 40ul peripheral blood which placed in 160ul diluent,using sysmex-2000i automatic blood cell counter to count the platelet number in whole blood and peripheral blood respectively,compared platelet number in venous blood with peripheral blood,and did blood smear of anticoagulant EDTA-K_2.Results:In health people,the platelet number in anticoagulant EDTA-K_2 compared with in diluted peripheral blood,there were no significant difference and were not statistically significant(P0.05).On Blood smears,the size,shape,distribution of platelet were normal.In EDTA-K_2-dependent PTCP patients,the platelet number in venous blood compared with in diluted peripheral blood,there were significant difference and were statistically significant(P0.001).There are large number of platelet aggregation on the end of blood smears and the number were different.Conclusion:Anticoagulant EDTA-K_2 can cause platelet count false decrease in some patients,but through platelet counts of diluted peripheral blood and peripheral blood smear,pseudothrombocytopenia can be corrected,and misdiagnosis can be avoided.
