摘要
目的:研究周围型肺癌时胸膜凹陷征的螺旋HRCT表现。材料与方法:搜集经手术、病理证实的周围型肺癌60例,根据肿瘤大小及生长部位,总结其胸膜凹陷的不同螺旋HRCT表现,并分析其形成的原因。结果:根据肿瘤发生的部位及其与胸膜结构的空间位置关系将周围型肺癌的胸膜凹陷征归纳为3种主要表现:(1)线条状影,近脏层胸膜面处见小三角形影;(2)典型表现,横断面上见一组有一定形态变化规律的胸膜凹陷改变,在3维SSD重建图像上脏层胸膜呈皱缩改变;(3)水平裂和斜裂胸膜凹陷,表现为曲线状。结论:螺旋高分辨率CT尤其是3D重建有利于周围型肺癌胸膜凹陷征的显示。
Objective: To evaluate the potential role of helical high resolution computed tomography (HHRCT) in assessment of pleural indentation (PI) associated with peripheral bronchogenic carcinoma.Materials and Methods:Sixty patients with peripheral bronchogenic carcinoma confirmed by pathology were collected in this series. The pleural indentation presented on HHRCT scans varied with the size and location of the lesion. The pathologic basis of pleural indrawing were briefly evaluated.Results:According to the location of the tumor, the findings of the PI could be divided as type A, having two or more lines extending to the visceral pleura and terminating in a small triangular density at the pleura; type B, typical PI; and type C, untapered straight or undulating lines of minor or major fissure.Conclusion: HHRCT, especially 3D reconstruction with surface shielding display, is helpful to show the pleural indentation in peripheral lung cancer.
