经皮椎体后凸成形术治疗骨质疏松性椎体压缩性骨折

Treatment of osteoporotic vertebral compression fractures with percutaneous kyphoplasty

摘要 目的:探讨经皮椎体后凸成形术(PKP)治疗骨质疏松性椎体压缩性骨折(OVCF)的临床疗效及相关并发症的预防。方法:在C型臂X线机透视下,应用PKP治疗11例OVCF。测定手术前后患者的视觉模拟评分、伤椎前缘高度及Cobb角,并作统计分析。结果:11例患者均手术成功,术后患者较术前视觉模拟评分减小、伤椎高度增加及Cobb角恢复(P<0.01)。术中出现1例骨水泥渗漏,但未出现肺栓塞和脊髓、神经受压。术后2个月出现1例相邻椎体骨折,无肋骨骨折、感染并发症发生。结论:PKP治疗OVCF能够迅速减轻骨折引起的疼痛,并能恢复伤椎高度,矫正脊柱后凸畸形,且手术创伤小,安全可靠。 Objective:To investigate the clinical effect of percutaneous kyphoplasty(PKP) in treatment of osteoporotic vertebral compression fractures,and the prevention of the complications.Methods:PKP was performed under C-arm X-ray in 11 cases of osteoporotic vertebral compression fractures.The visual-analogue scale,anterior vertebral height and Cobb angle were surveyed and analyzed before and after the operation.Results:The operation was successful in all the 11 cases.The visual-analogue scale,vertebral height and Cobb angle were restored after the operation.Bone cement leakage occurred in 1 case during the operation,but no pulmonary embolism or oppression to the spine or nerve was observed;one suffered adjacent vertebral fracture after the operation;no infection or rib fracture occurred.Conclusions:Percutaneous kyphoplasty is safe and effective for treatment of osteoporotic vertebral compression fractures,which may reduce the pain,restore the vertebral height and rectify the spinal kyphosis with little surgical trauma.
