代谢综合征病人颈动脉粥样硬化与C-反应蛋白、瘦素及脂联素关系 被引量:10

CORRELATION OF CAROTID ARTERY ATHEROSCLEROSIS WITH C-REACTIVE PROTEIN,LEPTIN AND ADIPONECTIN IN PATIENTS WITH METABOLIC SYNDROME

摘要 目的了解代谢综合征(MS)病人颈动脉粥样硬化与C-反应蛋白(CRP)、瘦素(Leptin)、脂联素(APN)及胰岛素抵抗的相互关系及其机制。方法根据双侧颈动脉超声检查结果,将226例MS病人分为无颈动脉斑块组(MS-1组,94例)及颈动脉斑块形成组(MS-2组,132例),测定两组病人血清胰岛素、超敏CRP(hs-CRP)、Leptin及APN,行双侧颈动脉超声检查半定量估计斑块的范围及严重程度。以36例健康体检者为对照组。结果 MS-1组及MS-2组的颈动脉内膜中层厚度(IMT)、胰岛素抵抗指数(HOMA-IR)、hs-CRP、Leptin均显著高于对照组(F=54.36~838.53,q=5.94~52.19,P〈0.01),APN则显著低于对照组(F=47.17,q=5.58~13.64,P〈0.01);MS-1组与MS-2组间各指标相比较,差异有显著性(q=5.76~61.35,P〈0.01)。IMT与HOMA-IR、hs-CRP、Leptin呈明显正相关(r=0.356~0.768,P〈0.01),与APN呈明显负相关(r=-0.682,P〈0.01)。结论 hs-CRP、Leptin及APN与MS病人颈动脉粥样硬化形成和胰岛素抵抗有关,可能是颈动脉粥样硬化的独立预测因子,可用于MS病人危险分层的评估。 Objective To investigate the correlation of carotid atherosclerosis(CAS) with C-reactive protein(hs-CRP),leptin,adiponectin(APN) and insulin resistance(IR) in patients with metabolic syndrome(MS),and their mechanism.Methods Based on the results of bilateral carotid artery echocardiography,226 patients with MS were divided into MS-1 group: patients without carotid-artery plaque(94 patients) and MS-2 group: those with carotid-artery plaque(132 patients).Serum insulin,hs-CRP,leptin and APN were detected.Bilateral carotid-artery ultrasonography was performed,and the extent and severity of the plaques were estimated by semi-quantitative method.Thirty-six healthy persons undergoing medical examination served as controls.Results Carotid intima-media thickness(IMT),IR index(HOMA-IR),hs-CRP and leptin in both MS-1 and MS-2 were significantly higher than that in the control(F=54.36-838.53,q=5.94-52.19,P0.01),and APN was significantly lower(F=47.17,q=5.58-13.64,P0.01).A comparison between MS-1 and MS-2 in terms of the above items showed significant differences among them(q=5.76-61.35,P0.01).IMT was positively correlated with HOMA-IR,hs-CRP and leptin(r=0.356-0.768,P0.01),while negatively correlated with APN(r=-0.682,P0.01).Conclusion hs-CRP,leptin and APN are associated with CAS and IR in MS patients,the parameters are likely to be independent predictors for CAS,which might be used for risk stratification in patients with MS.
