摘要
目的探讨参灵方对佐剂性关节炎(adjuvant arthritis,AA)大鼠药源性胃黏膜损伤的防治作用。方法建立大鼠AA模型,造模后将出现二次反应的大鼠随机取48只分为模型组、复合模型组、雷尼替丁组、参灵方小剂量组、参灵方中剂量组、参灵方大剂量组,加上空白对照组,每组8只,共计7组。雷尼替丁组予雷尼替丁30 mg/(kg.d)灌胃,参灵方小剂量组、中剂量组、大剂量组分别予参灵方3.5 g、7.0 g、14.0 g/(kg.d)灌胃,空白对照组、模型组和复合模型组分别予等容积0.9%氯化钠注射液灌胃,均为14 d。计算治疗前后大鼠足爪肿胀率、关节炎指数及胃黏膜损伤指数、各灌药组对胃黏膜损伤的抑制率、胃黏膜的病理学变化,并进行组间比较。结果给药前后模型组、复合模型组、雷尼替丁组、参灵方各剂量组组间关节炎指数、足爪肿胀率差异均无统计学意义(P>0.05);给药前后各组与空白对照组比较,足爪肿胀率差异均有统计学意义(P<0.05);参灵方治疗后大鼠足爪容积、关节炎指数较模型组、复合模型组均有所下降,但差异无统计学意义(P>0.05)。雷尼替丁组、参灵方小剂量、中剂量、大剂量组胃黏膜损伤指数与复合模型组比较差异均有统计学意义(P<0.01),参灵方小剂量、中剂量组与雷尼替丁组比较胃黏膜损伤指数差异无统计学意义(P>0.05),而参灵方大剂量组与雷尼替丁组比较差异有统计学意义(P<0.05)。雷尼替丁组、参灵方小剂量、中剂量、大剂量组胃黏膜损伤抑制率分别为74.05%、66.81%、74.41%、55.70%,参灵方小剂量组、大剂量组与中剂量组及雷尼替丁组比较差异有统计学意义(P<0.05)。结论参灵方对AA大鼠关节无明显影响,但对药源性胃黏膜损伤有保护作用。
Objective To investigate the effect of Shenling decoction against drug-induced gastric mucosa injury in adjuvant arthritis(AA) rats.Methods The complex rat models of AA and drug damage were rendered.After 2 times of reaction,48 rat models were randomly selected and divided into 7 groups: model group,compound model group,Ranitidine group,Shenling decoction low-dose group,Shenling decoction middle-dose group,Shenling decoction high-dose group,and blank control group(8 each).Ranitidine group underwent intragastric administration treatment with Ranitidine at a dosage of 30 mg/(kg·d),and Shenling decoction low-dose group,Shenling decoction middle-dose group,Shenling decoction high-dose group underwent intragastric administration treatment with Shenling decoction at different doses of 3.5 g,7 g,14 g/(kg·d)respectively.Blank control group,model group and compound model group were given isometric 0.9% sodium chloride injection.All the groups were treated for 14 days.Tumescence incident and index of foot and claw,and gastric mucosa injury index and inhibition ratio,and the pathologic changes were compared before and after treatment.Results There was no significant difference in joint tumescence incidents and indexes among model group,compound model group,Ranitidine group and Shenling treatment groups before and after treatment(P0.05).When compared with that in the blank control group,tumescence incident of foot and claw in the other groups showed significant difference before and after treatment(P0.05);the foot volume and arthritis index in the groups treated with Shenling decoction were lower than those in model group and compound model group without significant difference(P0.05).When compared with that in the compound model group,the gastric mucosa damage index in Ranitidine group,Shenling decoction low-dose group,Shenling decoction middle-dose group,Shenling decoction high-dose group showed significant difference(P0.01),but there was no significant difference among Shenling decoction middle-dose group,Shenling decoction high-dose group and Ranitidine group(P0.05).There was significant difference of gastric mucosa damage index between Shenling decoction high-dose group and Ranitidine group(P0.05).Inhibition ratios of gastric mucosa damage in Ranitidine group,Shenling decoction low-dose group,Shenling decoction middle-dose group,and Shenling decoction high-dose group were 74.05%,66.81%,74.41%,and 55.70% respectively,and there ws significant difference among the groups(P0.05).Conclusion Shenling decoction has protective efficacy on the drug gastric mucosa injury without significant adverse effect on AA rat's joints.
