速度向量成像技术定量评价冠心病患者心肌扭转运动 被引量:3

Quantitative assessment of left ventricular twist by velocity vector imaging in patients with coronary artery heart disease

摘要 目的:应用速度向量成像(velocity vector imaging,VVI)技术评价急性及慢性心肌缺血患者的心肌扭转运动。方法:对24名正常对照者(对照组)、25例慢性心肌缺血患者(心肌缺血组)及25例急性心肌梗死患者(心肌梗死组)进行超声心动图检查,并存储VVI二维动态灰阶图像,脱机分析左心室短轴观中基底部及心尖部各节段的收缩期旋转角度及速度、舒张期旋转速度。结果:①3组受检者的心肌扭转运动方向相同;②心肌梗死组梗死节段收缩期旋转角度及速度、舒张期旋转速度均显著低于对照组;③心肌缺血组部分缺血节段的舒张期旋转速度显著降低,但收缩期旋转角度及速度与对照组相比,差异无统计学意义。结论:应用VVI技术定量分析心肌扭转运动可能有助于检出和鉴别心肌缺血及心肌梗死,为评价局部心肌运动提供更多信息。 Objective To assess the left ventricular(LV) twist by velocity vector imaging(VVI) in patients with coronary artery heart disease.Methods Twenty-four normal subjects,25 patients with chronic ischemia and 25 patients with acute myocardial infarction(AMI) underwent echocardiography and had their VVI two dimensional dynamic gray scale images stored.The LV peak systolic twist velocity(Vsys),peak systolic twist(Tsys) and peak diastolic untwist velocity(Vdia) in apical and basal segments of short-axis view were measured by off-line.Results ① The direction of LV twist in all three groups was the same.② In AMI group,Vsys,Tsys and Vdia of infarcted segments were significantly lower than that of control group.③ In ischemic group,Vdia was significantly lower in some of the ischemic segments while Vsys and Tsys were not significantly different from that of control group.Conclusions As a new technique to quantify LV twist,VVI could be used to detect and differentiate ischemic myocardium and myocardial infarction,providing valuable information for the analysis of regional myocardial motion.
