老年无症状多发性腔梗患者认知功能与血小板聚集率之间的关系 被引量:7

Correlated study of cognitive in elderly patients with asymptomatic multiple cerebral lacunar infarction and rate of platelet aggregation

摘要 目的探讨服用阿司匹林的老年无症状多发性腔梗患者认知功能与血小板聚集率之间的关系。方法 2007年1月~2008年12月该科门诊及住院病人,经头颅MRI检查明确为多发性腔隙性脑梗死者,共72例,其中男44例,女28例。年龄65~81岁,平均年龄(72.6±6.5)岁。均已口服阿司匹林100 mg/d 1年以上。记录入选者的性别、年龄、血脂、高血压、冠心病、糖尿病、教育程度等情况,使用CHRONO-LOG血小板聚集仪测定ADP、AA诱导的血小板聚集率,由经过统一培训的医师进行简易精神状态量表(MMSE,满分30分)、蒙特利尔认知评估(MOCA,满分30分),记录评测结果。结果血小板聚集率与多发性腔隙性脑梗死患者得MMSE评分及MOCA评分均呈负相关(P<0.05),按照MMSE评分将入选对象分为MMSE正常组和MMSE异常组后,发现MMSE异常组ADP及AA诱导的血小板聚集率均高于MMSE正常组,但差异无显著性;按MOCA评分分为MOCA正常组和MOCA异常组后,发现MOCA异常组ADP及AA诱导的血小板聚集率均高于MOCA正常组,差异有显著性(P<0.05)。结论服用阿司匹林的老年无症状多发性腔梗患者,其认知功能减退与血小板聚集率升高相关。 【Objective】 To discuss the relationship with cognitive in elderly patients who took Aspirin and suffered from asymptomatic multiple cerebral lacunar infarction and the rate of platelet aggregation.【Methods】 72 patients(44 men,28 women) with a mean age of(2.6±6.5) years,who were all our patients from Jan 2007 to Dec 2008,participated in this study and definitely suffered from multiple cerebral lacunar infarction by the brain MRI.They have all been taking Aspirin 100 mg/d for at least one year.We recorded their conditions,such as sex,age,education,blood-fat,smoking and suffering from hypertension,coronary artery disease(CAD),diabetes or not,and so on.We determined ADP and AA-induced platelet aggregation rate with CHRONO-LOG platelet aggregation instrument,and used the MMSE and the MOCA to assess their cognitive functions and recoded.【Results】 The rate of platelet aggregation was correlated negatively with the score of MMSE and MOCA among these elderly patients with asymptomatic multiple cerebral lacunar infarction.According to the score of MMSE,we divided these patients into two groups: the normal MMSE group and the abnormal MMSE group.We disclosed that ADP and AA-induced platelet aggregation rates in the abnormal MMSE group were higher than those in the normal MMSE group.But we also found that result was not statistically significant.According to the score of MOCA,we divided these patients into two groups: the normal MOCA group and the abnormal MOCA group.We disclosed that ADP and AA-induced platelet aggregation rates in the abnormal MOCA group were higher than those in the normal MOCA group(P 0.05).【Conclusions】 Rising of rate of platelet aggregation was correlated with cognitive hypofunction in elderly patients who took Aspirin and suffered from asymptomatic multiple cerebral lacunar infarction.
