神经导航辅助切除功能区胶质瘤的体会 被引量:5

摘要 目的:总结神经导航系统辅助切除邻近重要功能区神经胶质瘤(简称胶质瘤)手术中的应用体会。方法:选择2006年11月至2009年11月我科住院确诊为邻近重要功能区神经胶质瘤的患者47例,采用Brain LAB公司VectorVision2导航系统辅助切除肿瘤,分析其手术方法及注意事项。结果:所有病例均在神经导航引导下顺利到达病变部位,病灶全切除27例(57.5%),次全切除15例(31.9%),部分切除5例(10.6%);平均注册误差为(1.4±0.5)mm;手术时间91~164min[平均(119.54±21.44)min];术后症状好转36例(76.6%),症状无变化7例(14.9%),症状加重或出现新症状4例(8.5%)。结论:神经导航系统仅仅是一个向导,一种辅助工具,受影像学资料的准确性、注册的准确性及术中影像漂移位等因素的影响,神经外科医师术中要时时注意防止和及时纠正系统的精确性,不能盲目依赖神经导航系统,需要具备神经解剖知识、娴熟显微手术技巧和丰富临床经验来完成手术。
