血糖、糖化血红蛋白与急性缺血性脑血管病相关性探讨 被引量:8

Exploration on the Relationship between Blood Glucose, Glycosylated Hemoglobin and Acute Ischemic Cerebrovascular Disease

摘要 目的探讨血糖(BG)、糖化血红蛋白(HbA1c)与急性缺血性脑血管病之间的关系。方法 89例急性缺血性脑血管病患者,其中短暂性脑缺血发作15例,脑梗死74例,根据有无缺血性脑血管病史,分为首发组和复发组。其中74例脑梗死患者中根据空腹血糖和既往有无糖尿病史分为糖尿病(DM)组、非糖尿病(NDM)组。并选取本院体检者56名为健康对照组。分析急性缺血性脑血管病时血糖、糖化血红蛋白对患者预后的影响。结果脑梗死组中血糖、糖化血红蛋白水平较短暂性脑缺血发作、健康对照组高;复发组血糖、糖化血红蛋白水平高于首发组;糖尿病组及非糖尿病高血糖组梗死容积均大于正常血糖组,糖尿病组神经功能缺损程度最重,正常血糖组治疗好转率较糖尿病组及非糖尿病高血糖组高(P<0.05)。其中糖化血红蛋白、糖化血清蛋白高对预后有明显影响。结论高血糖、高糖化血红蛋白与急性缺血性脑血管病相关,并影响急性缺血性脑血管病的预后。及时地控制高血糖,对缺血性脑血管病的治疗及预后有重要的意义。 Objective To explore the relationship between blood glucose (BG), glycosylated hemoglobin (HbAlc) and acute ischemic cerebrovascular disease. Methods 89 patients with acute ischemic cerebrovascular disease, including 15 cases of transient ischemic attack and 74 cases of cerebral infarction were divided into starting group and recurrent group according to with or without history of ischemic cerebrovascular disease. 74 cases of cerebral infarction patients according to fasting blood glucose and previous history of diabetes mellitus were divided into diabetic group and non-diabetic hyperglycemic group, and 56 cases of healthy physical examinationers in our hospital were selected as the control group. The influences of blood glucose and glycosylated hemoglobin on the prognosis of patients during acute ischemic cerebrovascular disease were analyzed. Results The levels of blood glucose and glycated hemoglobin in cerebral infarction group were higher than those in transient ischemic attack group and the control group;The levels of blood glucose and glycated hemoglobin in recurrent group were higher than those in the starting group;The infarcted volumes of diabetic group and non-diabetic hyperglycemic group were higher than that of normal blood glucose group;Diabetic group had the most serious neurologic impairment, and normal blood glucose group had higher improvement rate compared with diabetic group and non-diabetic hyperglycemic group(P0.05). Glycosylated hemoglobin and glycosylated serum protein had significant effects on the prognosis of patients. Conclusion High blood glucose and high glycosylated hemoglobin relate with acute ischemic cerebrovascular disease and affect the prognosis of patients. Timely control of high blood glucose has significant importance on the treatment and prognosis of ischemic cerebrovascular disease.
