p^(16)基因及其蛋白异常表达与肺腺癌病期及转移的关系 被引量:2

The aberrant expression of p16 gene and protein is correlated with the metastasis of lung adenocarcinoma

摘要 目的 探讨p16 基因及其蛋白异常表达与肺腺癌病期及转移的关系。方法 肺癌手术标本采用免疫组化 A B C法分析p16 蛋白的表达,其结果与p16 基因第2 外显子多聚酶链反应单链构象多态性分析( P C R S S C P)结果进行对比。胸水中肺腺癌细胞采用 P C R S S C P分析。结果 (1)74 例肺腺癌和76 例肺鳞癌中p16 蛋白阳性表达率分别为 47.3% 和47.4% ,二组间无显著差异( P> 0.05)。但肺腺癌p16 蛋白阴性表达频率与病期早晚和淋巴结转移部位密切相关;(2)p16 蛋白表达下调的主要原因为p16 基因纯合性缺失,点突变少见;(3)胸水肺腺癌细胞中p16 基因缺失的发生率(71.4% )显著高于Ⅰ期原发病灶(22.2% )。结论 p16 基因异常表达是肺腺癌发生的晚期事件,其异常表达与病期及转移密切相关。 Objective To evaluate the correlation of aberrant expression of P16 gene and its encoding protein with stage of disease and metastasis in patients with lung adenocarcinoma.Methods Immunohistochemical assay for p16 protein expression was carried out in resected non small cell lung cancers.The result was then compared with PCR SSCP assay for the exon 2 of p16 gene.The aberrant expression of p16 gene in adenocarcinoma cells within the pleural effusion was evaluated by PCR SSCP.Results Our data demonstrate that(1)the positive expression of p16 protein in 74 cases of adenocarcinoma and in 76 cases of squamous cell carcinoma was 47.3% and 47.4%,respectively,with no statistical significant difference between these two groups( P > 0.05 ).However,the aberrant expression of this protein in adenocarcinoma was significantly increased in late stages and in lesions with lymph node metastasis( P <0.01);(2)the homozygous deletion of p16 gene could accounted for the downregulation of p16 protein expression.The point mutation was a rare event in our cases.and (3)the incidence of p16 gene deletion was much higher in the adenocarcinoma cells within pleural effusiong(71.4%) than in those of primary lesions at stage Ⅰ disease(22.2%).Conclusion These results suggest that the aberrant expression of p16 gene is a late event in the pathogenesis of lung adenocarcinoma.This aberrant expression is closely correlated with metastasis.\;
