摘要
目的探讨3种肿瘤标志物(Tumor marker,TM)即甲胎蛋白(AFP)、癌胚抗原(CEA)、糖类抗原(CA19-9)在原发性肝癌早期诊断中的价值。方法应用电化学发光免疫分析法测定原发性肝癌组395例、对照组158例血清中此3种指标的含量,应用ROC曲线分析3种肿瘤标志物单独及联合检测的诊断价值。结果 3种肿瘤标志物在原发性肝癌组的浓度显著高于对照组(P<0.01)。在肝癌及对照组中,三者联合检测与二者联合检测的曲线下面积(AUC)差异不大,但都明显高于单一检测的AUC。结论通过ROC曲线分析,AFP+CEA或AFP+CA19-9联合检测具有良好的诊断价值。
Objective To investigate the diagnostic value of Alpha-Fetoprotein(AFP),carcinoembryonic antigen(CEA),CA19-9 in diagnosis of early primary hepatic carcinoma.Methods Electro-Chemiluminescence Immunoassay(ECLIA)was used to detect the serum level of AFP,CEA,CA19-9 of primary hepatic carcinoma group(395 cases)and control group(158 cases).The receiver operator characteristic curve(ROC curve)was used to analyze the diagnostic value of individual examination and combined examination of tumor markers.Results The serum level of AFP,CEA,CA19-9 in primary hepatic carcinoma group were significantly higher than that of control group(P〈0.01).The area under the curve(AUC)of AFP,CEA,CA19-9 were 0.909,0.900,0.896.The AUC of combined examination was larger than that of individual examination.Conclusion The combined examination of AFP+CEA or AFP+CA19-9 tumor marker has diagnostic value for primary hepatic carcinoma.
