摘要
目的探讨小儿消化道出血的临床特点,了解小儿消化道出血的病因。方法对我院2002年1月~2009年6月收治的426例消化道出血患儿的临床资料进行回顾性分析。结果 426例患儿中,男孩多于女孩;婴幼儿期发生率最高,其中以新生儿期多见;农村儿童发生率高于城市儿童。上消化道出血的临床表现以呕血多见,下消化道出血以便血多见。实验室检查以ECT检查阳性率最高,其次为胃镜检查。出血部位多为消化道局部病变所致。上消化道出血210例,病因依次为消化性溃疡,胃、十二指肠炎等;下消化道出血216例,病因依次为肠息肉、炎症性肠病、新生儿坏死性小肠结肠炎等。结论我院收治的小儿消化道出血涉及各年龄段,婴幼儿期发生率最高,其中以新生儿期多见,可能与我院新生儿科主要接收来自基层医院的危重病人有关;农村儿童的发生率高于城市儿童。儿童消化道出血的病因以消化道局部病变为主。上消化道出血以消化性溃疡及胃、十二指肠炎多见;下消化道出血则以肠息肉及炎症性肠病多见。
Objective To explore the clinical features of gastrointestinal hemorrhage in childrens patients in Guangxi.Methods A retrospectively study was performed 426 cases of hospitalized children with gastrointestinal hemorrhage from January 2002 to June 2009 in the First Affiliated Hospital of Guangxi Medical University.Results All of the 426 cases of children,the infants were more easy to suffered gastrointestinal hemorrhage,and most of them were neonatus;high incidence of rural children was higher than the urban children.The more common upper gastrointestinal bleeding was hematemesis.The more common of lower gastrointestinal bleeding was hemafecia.Supplementary examination：increasing the most positive rate examination was ECT.Followed by endoscopythe.Locations and causes of bleeding：More localized lesions were in the digestive tract;the upper gastrointestinal bleeding were 210 cases,most of them caused by peptic ulcer,various types of gastritis（such an superficial gastritis,erosive gastritis,hemorrhagic gastritis,gastric inflammation） and a total of 25 cases of duodenitis.The lower gastrointestinal bleeding were 216 cases,most of them with caused by intestinal polyps;followed by inflammatory bowel disease;necrotizing enterocolitis was in third place.Conclusion In our hospital,children of all ages involved in gastrointestinal bleeding.The incidence rate of rural children was high than urban children.The common locations of gastrointestinal bleeding in children were mainly localized gastrointestinal disease.Peptic ulcer and gastroduodenal inflammation were more common caused upper gastrointestinal bleeding.And the ower gastrointestinal bleeding is much more common for the intestinal polyps and inflammatory bowel disease.
