The value of ^(18)F-FDG coincidence SPECT in the patients with larynx carcinoma

摘要 目的:探讨18F-FDG符合线路显像在喉癌中的应用价值。方法:30例经病理证实的喉癌患者术前行18F-FDG双探头符合线路显像及CT检查。其中28例行手术治疗,以术后病理检查结果作为"金标准",比较18F-FDG符合线路显像及CT检查在原发肿瘤、淋巴结及远处转移(TNM)中的价值。结果:18F-FDG与CT探测淋巴结病变的灵敏度为75.00%和62.50%,特异性78.26%和43.48%,准确性76.36%和54.55%,阴性预测值69.23%和45.45%,阳性预测值82.76%和60.61%,其中特异性、准确性差异有统计学意义(P<0.05)1。8F-FDG符合线路显像发现2例有远处转移,改变了治疗方案。结论:18F-FDG符合线路显像在喉癌术前分期和治疗方案的制定中具有较好的临床应用价值。 Objective:Exact preoperative staging of larynx carcinoma is essential for accurate prognosis and selection of appropriate treatment.The aim of this study was to assess the value of 18F-FDG coincidence SPECT in the initial staging of patients with larynx carcinoma.Methods:18F-FDG coincidence SPECT and CT were performed and interpreted before operation in 30 patients with newly diagnosed larynx carcinoma.The results were then compared with the corresponding postsurgical histology in terms of tumor,nodal,and metastatic(TNM) disease status.Twenty-eight patients were finally operated: the rest 2 had distant metastases.Results:Based on 18FFDG coincidence SPECT analysis,the sensitivity in lymph node metastasis was 75.00%,the specificity 78.26%,with a diagnostic accuracy of 76.36%.The negative and positive predictive value of 18F-FDG coincidence SPECT was 69.23% and 82.76%,respectively.CT resulted in a sensitivity of 62.50%,specificity of 43.48%,and accuracy of 54.55% .The negative predictive value of CT was 45.45% and the positive predictive value 60.61%.There were significant differences in specificity,accuracy,and positive predictive value between 18F-FDG coincidence SPECT and CT.Conclusion:18F-FDG coincidence SPECT can be used as an important tool for the initial staging and treatment planning of the patients with larynx carcinoma.
