绿脓杆菌耐药性及β-内酰胺酶基因的研究 被引量:1

摘要 对72 株临床分离的绿脓杆菌进行了药敏试验, 其中42 株检测了β-内酰胺酶及几种β-内酰胺酶基因同源序列在总DNA和质粒中的情况,并探讨了绿脓杆菌总DNA中不同β-内酰胺酶基因同源序列与不同β-内酰胺类抗生素耐药性的关系。结果显示, 绿脓杆菌有普遍耐药性和多重耐药性, 在42 株绿脓杆菌中有34 株产β-内酰胺酶, 阳性率80.95% , β-内酰胺酶基因同源序列在总DNA中的检出亦有普遍性和多重性的特点, 42 株菌总DNA 中检出PSE-1、TEM-1、SHV-2、OXA-1、OXA-2β-内酰胺酶基因同源序列2~4种不等, 其中有17 株菌分离到质粒(17/42), 这17株菌的质粒均检出1~2 种β-内酰胺酶基因同源序列不等。从酶基因的类型看,染色体介导的以PSE-1型为主,质粒介导的以TEM-1 型为主。Logistic回归分析显示β-内酰胺类抗生素耐药性与β-内酰胺酶基因的关系有3 种情况: (1) 与一种特定的β-内酰胺酶基因有关; (2) 与几种β-内酰胺酶基因有关; (3) 与β-内酰胺酶基因无关。 Seventy two strains of Pseudomonas Aeruginosa were isolated clinically from nosocomial infection cases in Tongji Hospital. Antimicrobial susceptibility test was carried out for every isolation The β lase and β lactamase genes of total DNA and plasmids in 42 isolates of P. aeruginosa were detected. The relationship between the different β lactamases gene and resistance of P. aeruginosa to different antimicrobial agents was studied. The results showed that P. Aeruginosa possessed the characteristic of general and multiple drug resistance. The β lase was found out by Nitrocefin in 34 strains out of 42 isolates with the rate of positive reaction being 80 95%. The existence of the β lactamase genes also had widespread and multiple characteristics. Two to 4 β lactamase genes of the PSE 1, TEM 1, SHV 1, OXA 1 and OXA 2 type in total DNA of 42 P. Aeruginosa were demonstrated. Plasmids were detected in 17 strains of P. aeruginosa (17/42). These strains harbored 1 to 2 β lactamase genes in the plasmid. Southern analysis showed that the PSE 1 type was dominant in the β lactamase genes of chromosome mediated. The majority of genes are the TEM 1 type in the β lactamase genes of plasmid mediated. The logistic analysis displayed the resistance of P. aeruginosa to different β lactam agents might depend on β lactamase differently: (1) It is affected by the spectial type of β lactamase gene. (2) It is influenced jointly by the many β lactamse genes. (3)It is not attacked by the β lactamase gene.
