摘要
目的:探讨四君子汤对喂养不耐受早产儿血清胃泌素(GAS)、血浆胃动素(MOT)的影响。方法:将临床诊断为喂养不耐受的早产儿(均为29~35周)68例随机分为治疗组34例及对照组34例。对照组按照早产儿管理指南规范诊疗的同时,喂养不耐受治疗期间,每次喂奶前予非营养性吸吮10 min,并在喂奶前30 min口服或鼻饲多潘立酮混悬液和静脉给予红霉素。治疗组在对照组基础上予口服四君子汤中药汤剂。在喂养不耐受治疗前后,应用放射免疫法批量检测2组病例的血清GAS、血浆MOT水平,进行自身前后对照与组间对照统计学分析。结果:2组患儿血清GAS、血浆MOT平均水平治疗后比治疗前明显升高(P<0.05或P<0.01),且治疗组水平明显高于对照组(P<0.05)。2组肝功能、肾功能及生命体征未发现明显异常。结论:血清GAS及血浆MOT的水平可以反映胃肠功能成熟的程度,四君子汤可提高喂养不耐受早产儿血中GAS、MOT水平,促进其胃肠功能的成熟。
Objective:To explore the effect of Si Junzi Decoction on serum gastrin(GAS) and plasma motilin(MOT) in premature infants with feeding intolerance.Methods: 68 cases of premature infants with feeding intolerance were randomly divided into treatment group and control group.According to management guidelines for premature infants,the control group infants were given conventional treatment.During the treatment of feeding intolerance,they were given 10 minutes of non-nutritive sucking before each feeding.What's necessary is additional treatment with Domperidone Suspl and intravenous Erythromyc,30 minutes before feeding.And the treatment group,after being treated as the control group,was given oral Si Junzi Decoction.Before and after treatment in feeding intolerance,mass detection was made by radioimmunoassay of GAS and MOT levels of the cases in 2 groups.Comparison and statistic analysis were made.Results: Serum GAS and MOT average plasma of the 2 groups patients after treatment were significantly higher than that before treatment(P0.05 or P0.01),and they were significantly higher in the treatment group than in the control group(P0.05).There were no obvious abnormal signs in occult blood + stool test,liver function and kidney function of the two groups.Conclusion: Serum levels of GAS and plasma MOT gastrointestinal function may reflect the degree of maturity;Si Junzi Decoction can increase GAS and MOT levels of children with feeding intolerance and promote the maturation of gastrointestinal function.
