摘要
目的探讨国产高纯度尿卵泡刺激素(丽申宝)在体外受精-胚胎移植(IVF-ET)周期应用的临床结局。方法对2008年3月至2009年12月在本中心进行IVF ET治疗,并完成胚胎移植的279例患者307个控制性超排卵(COH)周期临床资料进行回顾性分析。按COH中使用的不同促性腺激素(Gn),患者分为果纳芬组(1 66例、183个周期)和丽申宝组(113例、1 24个周期),比较两组在用药时间、用药剂量、药品费、获卵数、临床妊娠率、中重度卵巢过度刺激综合征(OHSS)发生率、妊娠结局及围生儿结局的差异。结果两组用药时间、用药剂量、获卵数、成熟卵数、优质胚胎率及OHSS发生率均无显著差异;两组丽申宝组临床妊娠率(41.1%)、种植率(25.1%)、流产率(11.8%)、多胎率(33.3%)及抱子回家率(33.9%)与果纳芬组(分别为50.8%、31.4%、18.3%、32.3%、36.1%)相比亦无显著性差异;出生婴儿的胎龄、出生体重及出生缺陷均无显著性差异;而药品费用丽申宝组(5,092.68±1,388.98)明显低于果纳芬组(8,226±2,716.33),有显著性差异(P<0.05)。结论丽申宝具有与果纳芬相同的促排卵效应和临床结局,但药品费显著低于果纳芬。
Objective： To study the efficacy and safety of highly purified human urinary follicle-stimulating hormone （HP-hFSH） during controlled ovarian stimulation （COH） in patients undergoing in vitro fertilization and embryo transfer （IVF-ET）. Methods： A total of 307 IVF/ICSI cycles in 279 infertile couples were divided into two groups according to the FSH types used （124 cycles of 113 patients in HP-hFSH group and 183 cycles of 166 patients in recombinant FSH group）. Parameters concerning clinical efficacy and safety were compared between groups. Results： There was no significant difference in duration of COH, total dose of FSH used, number of retrieved oocytes, number of retrieved oocytes, rate of good-quality embryos, clinical pregnancy rate, implantation rate, abortion rate, incidence of ovarian hyperstimulation syndrome （OHSS） or multiple pregnancy rate between two groups. No significant differences in term pregnancy, birth weight, birth defects or taken-home-baby rate were found either. However, the cost of gonadotropin in HP-hFSH group was significantly lower than that in recombinant FSH group. Conclusions： HP hFSH could achieve similar COH efficacy and clinical outcomes to recombinant FSH in IVF treatment with the advantage of lower cost.
